Under direct supervision of the Human Services Supervisor – Public Health, this position supports the mission of Blue Earth County Human Services by effectively and efficiently delivering essential services to families and individuals so they can achieve their potential through active community participation. This position promotes, protects, and preserves the health of county residents through a variety of programs and activities. The Public Health Specialist - IMMTRACK Coordinator/MnCHOICES Assessor will provide leadership to the IMMTRACK immunization registry, assess community and provider needs to develop program planning to respond to needs and evaluate program activities. This position will also perform MnCHOICES assessments, maintain current and build new community partnerships, perform administrative duties, assist with public information, and assist in coordinating activities of program support staff.
EXAMPLE OF DUTIES
ESSENTIAL FUNCTIONS: Assures completion of integrated community assessments of local public health vaccine preventable diseases, infectious disease threats, and other similar threats to public health. Assesses and prioritizes community and provider needs related to immunization practices. Participates in registry activities and knowledge of immunization-related issues related to infectious disease threats. Develops program goals and objectives based on assessment findings.Plans and Implements Programs. Establishes a work plan to meet program goals and needs of registry partners, funding sources, and participants. Organizes and works with other professionals and community groups to assure the delivery of registry goals. Evaluates programs. Schedules and coordinates program-related meetings, presentations, and outreach activities. Actively maintains relationships with internal and external partners, community groups, providers, and other related professionals and organizations. Provides support for vaccine registry reporting, investigation, and efforts in the community and region. Assists in coordinating program-related activities of IMMTRACK support staff and students.Provides Leadership and Collaborates with Registry Partners Throughout the IMMTRACK Region. Collaborates and works with existing groups to address registry goals, assumes a leadership role in the communities to enhance and strengthen existing partnerships. Identifies potential new partners. Develops and implements strategies to facilitate new partnerships.Performs grant management through researching, preparing, and submitting grant applications and reports. Develops clear, concise, and coherent written plans that clearly describe existing problems, unmet needs, and future programmatic remedies. Prepares and submits grant reports. Determines and coordinates evaluation methods for implementation and reports on program activities. Maintains program files for quality assurance.Assists with public information and media relations activities. Handles print material such as public service announcements, news releases, reports, correspondence, etc. Designs brochures, overheads, and handouts. Designs and prepares display boards, posters, etc., for presentations and displays at community events. Serves as a resource to other groups to assist them in gathering immunization-related data. Represents IMMTRACK on committees and maintains knowledge of community resources.Responds to emergencies as directed by supervisor and participates in disaster support, communicable diseases, outbreaks, and investigations in whatever role assigned or designated. Works with communities to provide support in areas of need. Assists with planning, consulting, investigating, and providing technical assistance.Performs MnCHOICES assessments and intake for new and existing clients. Determines client eligibility for services through analysis of the information collected within agency and program eligibility criteria. Collaborates with internal and external partners to determine eligibility. Performs record keeping activities to maintain case files in accordance with external and internal program requirements. Performs Public Health Functions. Uses public health theories and processes to practice. Assesses health service needs of individuals and groups. Formulates plan/intervention based on data collection and assessment. Participates in preventative health care and health promotions. Participates in quality improvement and assurance processes. Promotes a diverse, culturally competent, and respectful workplace.Interacts Positively with Internal and External Customers. Communicates and interacts effectively and respectfully with employees, supervisors, individuals from other organizations, and customers.Performs the physical and mental demands of the Position.Attends Work During Regularly Scheduled Hours.ADDITIONAL FUNCTIONS: Participates in Professional Staff Development. Participates in training and education opportunities to maintain and improve proficiency. Attends and participates in professional organizations. Reads appropriate journals and articles associated with agents and departments from other counties and states to learn about and maintain professionalism. Performs Other Related Functions as Apparent or Assigned.
SUPPLEMENTAL INFORMATION
Monday through Friday from 8:00 a.m. to 5:00 p.m. Occasional evening and weekend hours may be required.
