New Horizons Healthcare (NHH) is seeking a fulltime, licensed Behavioral Health Counselor (BHC) to serve as a member of our integrated care services team. The NHH BHC will provide patient-centered, culturally sensitive mental health and substance use services to patients and their families in order to increase functioning and promote better health outcomes and wellness. As a member of the clinical services team, the BHC will work directly with the primary care providers and others on the care team to assess and treat patients through a brief, team-based, whole person care approach.
New Horizons Healthcare is a federally qualified health center (FQHC) in Roanoke, Virginia that provides comprehensive primary care, behavioral health, dental care, and pharmacy services for almost 10,000 patients annually. We are a not-for-profit community clinic with a mission to improve access to affordable, high quality, culturally competent, comprehensive primary health care. We offer a sliding fee discount program for those who are uninsured or underinsured.
New Horizons offers a wide array of integrated behavioral health services, including primary care behavioral health, outpatient therapy, medication assisted treatment, and case management. If you want to make a difference, this is an excellent opportunity!

Job Responsibilities:
• Provide screening and assessment of behavioral health, substance use, and other psychosocial issues.
• Provide effective treatment planning and interventions to assist patients to increase functioning and to achieve their health and wellness goals.
• Evaluate crisis situations and assist other team members when a higher level of care is needed.
• Collaborate with NHH’s other BH team members for care management, care coordination, and other patient care needs.
• Assist the primary care team to develop integrated disease management approaches that involve health behavior change and provide patient education to improve self-management of chronic disease.
• Provide consultation to the primary care providers regarding behavioral health for treatment planning and to increase health outcomes.
• Plan and provide group psychoeducation and group interventions.
• Identify and share prevention techniques and behavioral health practice guidelines with others at NHH.
• Actively participate in meetings that support NHH’s integrated health care program development, collaboration, continuity of care, and quality improvement.
• Monitor NHHs behavioral health integration program, identifying recommendations for improvements and training.
• Participate in development and administration of data and measures to assess multiple outcomes, to include patient and provider satisfaction, practice improvement, patient health outcomes, etc.
• Maintain clinical licensure and increase relevant and identified competencies through CEUs and other training opportunities.

• Ability to make quick and accurate clinical assessments of behavioral health concerns.
• High comfort in working with primary care medical providers in a team-based care approach.
• Ability to design and implement clinical pathways and protocols for behavioral treatments.
• Willingness to learn and apply brief, effective evidence-based behavioral health interventions (individual, family, and group).
• Ability to work effectively as a team, interfacing with patients, primary care providers, consulting psychiatry prescribers, case managers, and other members of the healthcare team, as well as with administrative and support staff.
• Ability to effectively represent NHH’s interests in the community and maintain effective working relationships among co-workers, community stakeholders, and professional groups.
• Willingness to participate in required organizational initiatives, activities, and events.
Required Qualifications:
• Degree and current license as a psychologist, LCSW, LPC, or LMFT. Licensure-eligible will be considered.
Preferred Qualifications:
• Prior work experience in a healthcare or other integrated behavioral health system.
• Prior work with diverse patient populations.
• Bilingual in Spanish and English.
• Prior work or training in the primary care behavioral health (PCBH) model.
