Friendship is seeking a Full Time MDS Coordinator (RN) to join our Friendship – North team, located on Hershberger Road. The MDS Coordinator is responsible for maintaining RAI regulations and principles while completing MDS assessments and care planning for residents. The position is responsible for optimizing PDPM, and RUGS 48 reimbursement. The MDSC will assist with Managed Care updates as needed.
Roles & Responsibilities:
• Maintain guidelines as per RAI manual in timely completion of MDS and care planning.
• Knowledge of PDPM, RUGS 48, Managed Care, and Medicare Replacement Policies.
• Use data from resident’s record, enter correct/appropriate answers on the computerized MDS
• Assist TEAM in Baseline CP implementation and Review with Resident/Resident RP, as per Federal Regulations.
• Provide Managed Care updates to Case Mangers as may be indicated.
• Analyze aspects of resident’s individual needs and facilitate collaboration of the interdisciplinary team to develop Resident Centered Care Plan, to enable resident to achieve their highest level of functioning.
• Coordinate with the Interdisciplinary team members in timely completion of the MDS.
• Must attend facility mandated in-services.
• Assist in maintaining facility readiness for all regulatory inspections including Virginia Department regulations,
• Work as a liaison between all specialty physicians and physician offices for any related questions, concerns, including rounding with physician if needed and preparing weekly patient list,
• Adhere to all policies and procedures set forth by Friendship and conveys these policies to nursing staff.
• Assist with UAI Completion as may be indicated.
• Assist with ADR’s,
• Knowledge of Quality Measures.
Qualifications:
• Nursing Degree from an accredited institution or equivalent graduation from an approved RN program, required
• RN with MDS 3.0 knowledge – certification preferred (AAPACN or comparable agency)
• Experience in Long-term Care environment preferred
