With consultation and reporting to the director of nursing or administrator, the nurse is responsible for total patient care within the nursing
unit assigned to and management of the assigned wing. Tasks performed by the nurse are as followed: observes and reports symptoms and conditions of patients. Takes and records temperature, respiration’s, and pulse. Administer medications as prescribed by the physician and treatments. Bathes and feeds patients as needed. Assists in maintaining a physical, social, and psychological environment which will be conducive to the best interest and welfare of the resident’s condition. Is responsible for following the charting regimen established by the D.O.N. is responsible ordering or reordering medications from pharmacy. Assists with careplanning for residents. Keeps bedside careplan up to date. Supervises can’s. Follow pharmacy and lab protocol.
Responsibility for:
Providing suitable environment for residents. Patient care duties as directed. Gives and receives reports, answers telephone inquiries. Evaluates resident’s needs. Condition and care. Assists in developing nursing care plan for individual residents including rehabilitated and restorative activities, and instruction in self-help. Admits and orients activities, and instruction in self-help. Admits and orients new residents and relatives with D.O.N. direction. Takes physician’s orders, transfer and pre-admission information as needed. Revises resident’s chart, checks to see that all required forms are signed. Transcribes physician’s orders to patient bedside careplan, medication and treatment cars, diet sheet. Periodically reviews resident care plans with the care planning team members. Reports equipment that is not in good working order. Maintains neat records. Performs charting duties as established. Maintains narcotic records. Completes required unit or medical chart forms. Completes nurse progress of resident’s condition. Schedules appointments and arranges for transportation of residents as needed for diagnostic, therapeutic, dental or medical services. Assists in establishing good public relations. Participates in in service training meetings. Continues professional growth through current literature, professional meeting, institutes and work shops. Assists in completing M.D.S (minimum data set). on residents initially, quarterly, annually as needed. Attends inservice and staff meetings. Directs and supervises functions and personnel (nursing) quality of patient care. Compliance with established policies, procedures and state/federal regulations. If residents are to be transferred to another institution, arranges for this transfer and completes required forms. Oversees that resident is escorted to transportation or transfer vehicle. Performs and/or supervises and assists with patient care according to outlined policy and procedure, such as a.m. and p.m. care. Checks resident’s chart for specific treatment and medication orders. Checks resident’s daily schedule. Carries out or assigns medication orders. Carries out or assigns duties to other nursing personnel. Makes rounds, takes and records
Treatments, and performs other professional services needed, applying and changing dressings, bandages, packs, colostomy and drainage bags, etc., Massage and ROM exercise, isolation set – up and care. Care of the dead and dying and any other professional services within the scope on an L.P.N. gives emergency treatment when required, and notifies physician of emergency, takes and carries out physician ‘s orders. Accompanies physician on rounds and assists with examinations and treatments as needed. Notifies physician of automatic stop orders on specific medications. Notifies physician of changes in resident’s condition, and any unusual or abnormal observations. May insert iv and observes iv site after obtaining doctor’s order. Care of residents that have feeding tubes such as and not limited to gastrostomy tube or nasogastic tube.
Physical demands:
Good physical and mental health. Must be able to withstand strenuous physical activity. Manual dexterity to handle instruments and equipment. Visual and hearing acuity to detect changes in resident’s condition.
Special demands:
Must have a genuine interest in geriatric nursing. Must have a liking for people and a desire to serve those who are ill and incapacitated. Ability to provide close supervision, and to give verbal and written instructions to those employees that the nurse is responsible for dealing with varying situations. Must use tact and courtesy in dealing with staff, residents, their families, and visitors. Patience and compassion is essential. Ability to keep resident’s information confidential. Must be able to perform job task with frequent interruptions.
Qualifications:
Education : shall be licensed as a graduate practical nurse by the board of examiners of practical nurses and hold current license in the state of Kentucky.
Training and experience : on-the –job training and completed the prescribed licensed practical nurse training.
Job knowledge :
Daily hygienic care of residents. Techniques of moving people and assisting them to and from bed serving and feeding people. Techniques of administering medications and treatments. Preventing spread of disease by proper disposal of infected materials and cleaning of equipment. First-aide assistance to render in emergencies. Care o household equipment and operation of appliances. Ability to manage and supervise a nursing unit. Has training in rehabilitative nursing. Has geriatric assessment skills. Has IV certification. It is the responsibility of the nurse to maintain required CEU's for Kentucky licensure every 2 years and maintain CPR certification
