Variable positions open within our At Home spaces, including with our Visiting Nurses Association, Home Care division. Nurses are assigned a zip code within a territory that allows for less driving and more time with each patient.

Days, Nights, 8 hour and 12 hour shifts

Job Summary:
Home Care Skilled Nursing through home visits. Great daytime hours and reasonable commutes allow for our Visiting Nurses to see up to 5 patients per day. Nurses are assigned a zip code within a territory that allows for less driving and more time with each patient.
Essential Functions
• Communicates effectively with team, patient and families. Assesses, develops, and maintains an individualized patient plan of care with the patient.
• As needed, specified by Corewell Health (formerly known as Spectrum Health) entity: Performs Medication Administration Pass according to regulatory guidelines. Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.
• Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
• Evaluates patient’s and significant other’s educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.
• Observes and reports the patient’s condition, needs and outcomes of intervention to the physician and other team members. Identify problems and emergency situations, and initiate immediate “life saving” measures in the absence of a physician.
• As needed, specified by Corewell Health entity: Teaches and supervises the care provided by CNA, LPN or other nursing service personnel according to regulatory and agency guidelines.
• Implements the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating the patient’s progress toward established goals recommending revisions to the plan as necessary, and planning for termination of service or discharge.
Qualifications
• Associate's Degree Nursing - Required
• Bachelor's Degree Nursing - Preferred
• 1 year of relevant experience of bedside acute care, emergency, rehab facility or home care experience -Preferred
• Registered Nurse (RN) in State of Michigan required upon hire
• Michigan Driver's License required upon hire
• Basic Life Support - required within 90 days
