PRIMARY PURPOSE:
The Nurse Navigator functions as a member of the multidisciplinary team of care providers who demonstrate a culture of cooperation, enthusiasm and respect. They are responsible for working collaboratively with the Value Based Performance team, the Clinical Transformation/Primary Care departments, Provider practices, and the patients directly. The Nurse Navigator is responsible for coordinating care of patients to enable a seamless experience for both patient experience and provider-office workflows. The Nurse Navigator is to aid patients through the screening process by providing continuity of care and support through all phases of screening and diagnosis. These workflows are accomplished through centralized telephonic, tangible, and electronic patient messages.
The Nurse Navigator's focus includes, but is not limited to, the following areas:
• Pre-visit patient outreach
• Appointment scheduling
• Care gap outreach and intervention
• Patient education
• Post-visit follow-up
• Order management
• Abnormal results communication
Fulfilling these responsibilities requires the Nurse Navigator to ensure that all pertinent medical information and care needs for patients are identified, documented, and communicated to providers if necessary.
The Nurse Navigator is responsible for promoting a culture of accountability and transparency across all SEP offices.
EDUCATION:
Minimum: Associate's in Healthcare related field with active KY RN Licensure
Desired: Bachelor's in Healthcare related field with active KY RN Licensure
YEARS OF EXPERIENCE:
Minimum: 2 years of experience in a medical setting
Preferred: 5 years of experience in a medical setting
REQUIRED SKILLS AND KNOWLEDGE:
• Excellent verbal and written communication skills.
• Analytical Skills
• Skills in establishing and maintaining effective working relations with physicians, office employees, and patients.
• Self-directed and performs duties willingly and proactively. Continuously strives for excellence.
• Should attain working knowledge electronic health record applications. Previous use of EPIC preferred.
• Ability to manage and prioritize multiple tasks.
• Positive Attitude
• Flexibility and adaptability to change.
• Must work carefully and precisely with attention to detail.
• Requires proficient computer skills especially Word, Excel, PowerPoint, Teams, and Outlook.
• Demonstrated ability to work successfully in a team-based decision-making culture.
• Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
• Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
• Must respond and follow through to requests from customers promptly.
• Must work carefully and precisely with attention to detail.
• Must utilize resources wisely.
• Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same.
• Cooperates with other departments and work groups.
LICENSES AND CERTIFICATIONS:
Minimum: Active RN license in the state of Kentucky. Must have a valid driver's license for travel between office sites
