Job Description Licensed Practical Nurse (LPN) Qualifications: * Active LPN license in the state of Minnesota * The ability to pass a background check * Must possess and valid driver's license with a good driving record * Strong verbal and written communication skills * Basic computer skills Responsibilities: * Licensed Practical Nurse is responsible for conducting a focused assessment of the health status of an individual, and reporting changes and responses to interventions * Responsibilities also include implementing interventions and assigning and monitoring nursing tasks or activities to unlicensed assistive personnel; promoting a safe and therapeutic environment, advocating for the best/safe interest of the client * Collaborating and communicating with client's health care provider's and team * Providing health care information to the client's staff members and management * Providing input in the development if policies and procedures * Respond and direct medical and behavioral emergencies * Coordinating appointments the week prior with the house manager and staff to ensure consultation forms are in locations prior to the appointment * Administering injections necessary to be done by nursing when applicable * Diabetic care coordination * Set up AMV and ride coordination for all client's prior to any health appointments and/or procedures * Assist Administration with any training, as needed- New hire medication, Nursing training, First Aid, etc. * Attend Psych appointments and other health appointments as needed * Attend any care meetings, administrative meetings as needed or required and be willing to give a nursing update pertinent to the specific client * Send out team emails after any and all appointments * Visit locations and assess client overall health * Set up client appointments, labs as needed and enter them on Therap * Follow up on Lab work and communicate with health care providers, care teams and administration * Train staff on any new medication started and ensure that the medication is being given if not set in the Dispill packs from Genoa Pharmacy * Therap management as needed with Admin manager and General manager- Ensuring MAR is being documented in and completed for the week at each location * Nail and Foot cares for Diabetics * Medication Administration Record configuration competency completed at the beginning of each month * Monthly vitals, height and weight and then documented in Therap * Ordering Nursing supplies and client supplies as needed such as; Depends, wipes, wound supplies, etc. * Attending client intake- configuring client MAR, setting up initial appointments and setting up medications with Genoa Pharmacy * Health Report Summary's for any team meetings prior to the meeting scheduled
