Join the St. Elizabeth team and enjoy a competitive compensation and benefits package, a great place to live and work and a very substantial referral base.The Advanced Practice Provider is primarily responsible for providing patient care services, including assessing, diagnosing, prescribing, treating, and educating patients. The Advanced Practice Provider works closely with physicians and other clinicians in a team approach to patient care. The Advanced Practice Provider is a highly visible position that is always responsible for creating a positive impression with patients, physicians and other visitors he/she encounters, both in person and on the phone. Responsibilities Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. Assures that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members. Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols. On call work Qualifications Demonstrated skills and knowledge as a Nurse Practitioner in area of expertise: One (1) year nurse practitioner experience One (1) year hospice or palliative nursing experience Excellent observation, verbal, and written communication skills, problem solving skills, mathematical skills, and nursing skills. Knowledge of community, professional and education resources. Knowledge of advanced practice including anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities Knowledge of considerable autonomy, and collaboration with multidisciplinary teams, physicians/Medical Director and others involved in patient's care directly and indirectly. Must coordinate care with the program staff, outreach areas (as applicable), family members, and work collaboratively with all physicians involved with patient's care directly and indirectly. Requires travel in own vehicle as needed. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment. Knowledge of health promotions, health risk identification, and patient education principles and techniques. You must be able to obtain a state license in KY and/or IN for your scope of practice.
