This is a clinical position to assist interdisciplinary teams (IDT) in developing, implementing, and evaluating patient centered treatment. The Oregon State Hospital (OSH) is a psychiatric hospital under the direction of the Oregon Health Authority (OHA) that inspires hope, promotes safety, and supports recovery. OSH employs over 2,000 FTE and serves over 600 patients each day on two campuses. OSH provides therapeutic, evidence-based, patient-centered treatment focusing on patient recovery and community reintegration. The TCP Specialist is a core member of the treatment team who shall perform the following key functions; coordination of all IDT meetings, facilitate the development of each treatment care plan (TCP), and integration of all aspects of patient care into the TCP including Behavior Support Plans, Emergency Treatment Care Plans, STARTs, and other key assessments.Interdisciplinary Team (IDT) Coordination:Coordinate schedule for all unit treatment care plan reviews including the patient, team members, family members, relevant community partners and all other needed parties, considering sick leave, vacations, and other scheduling considerations.Organize treatment care plan meetings, including keeping the team on track with the agenda.Assist the team in completing all sections of the Treatment Care Plan.Be the point of contact for the treatment mall manager, mall clinicians, and all other consultative IDT members regarding specific changes to a patient's treatment care plan.Develop processes and timelines for submission of discipline specific work prior to an IDT meeting to provide the IDT with all relevant information in the event of a team member absence.Work with all disciplines and OSH Senior Management to improve treatment care planning and communicate IDT needs to Senior Management. Work collaboratively with IDT Coordinators on other units to ensure standardization of IDT processes across all units.Assist the team in prioritizing, organizing, and accomplishing duties related to IDT processes and TCPs.Educate the team on hospital policies, standards, processes, and expectations related to Treatment Care Planning.Communicate with patient families and/or guardians related to Treatment Care Planning or IDT attendance.Meet with patients to resolve grievances. Help with treatment groups 2 hours a week.Treatment Care Plan Content Monitoring:Incorporate all consultations, assessments, behavioral support plans, etc. into the treatment care plan.Submit or assist in submission of Risk Assessment and Risk Review documentation to the IDT in a timely manner; include incident review forms, transfer tracker, and hospital level of care assessments.Ensure that team is following hospital policies, and expectations regarding the content of patient treatment care plans, timelines, and implementation of treatment care plans.Gather data regarding patient attendance and progress in treatment mall groups and report out to the team during treatment care plan reviews.Ensure IDT follows OSH documentation policies and procedures for patients on behavioral precautions.Develop and maintain systems to ensure IDT has all applicable information related to a patient during TCP reviews. Ensure clinical needs of patients are properly addressed in treatment care plans.Evaluate data on restrictive events, patient aggression, patient and staff injuries. Identify trends and make recommendations on TCP changes. Develop and implement policies and timelines for submission of discipline specific work, to ensure IDT has all needed information in the event of a team member absence. Develop detailed plans, goals and objectives for the implementation of changes to IDT's and TCPs.Quality Assurance and Data Analysis:Lead Quality Assurance projects related to the IDT as requested by supervisor.Lead IDT improvement projects related to IDT's and treatment care planning as requested, including content improvement projects and systematic improvement projects.Monitoring Team Functioning:The TCP Specialist will monitor the following aspects of team functioning and communicate any staff performance concerns to the Associate Clinical Director and/or applicable Discipline Chief. TCPS will be required to report to their assigned campus for at least 80% of their work week, or 4 out of their 5 working days per week.What we are looking for:A Bachelor's Degree in Business or Public Administration, Behavioral or Social Sciences, or a degree related to the agency program that demonstrates the capacity for the knowledge and skills; and two years' experience coordinating or administering a program.ORAny combination of experience or education equivalent to five years of experience that typically supports the knowledge and skill requirements listed for the classification.The ability to be responsible for setting IDT priorities to meet set standards, critical decision-making regarding content of TCPs and IDT functioning. Judgement and decision making in key logistics, including the timeliness and staff performance related to IDT's and treatment care planning.
