Full time General In Patient Hospital based-Lewis Gale Medical Center. Educating the hospital at large on hospice eligibility and benefits regarding both inpatient and outpatient hospice services. Developing long lasting and valuable relationships with providers and nursing team members to increase hospice referrals to ProMedica. Discussing end of life options and care with patients and families to provide a higher level of comfort and quality of life. Collaborating daily with ProMedica inpatient palliative care team to identify anyone needing a higher level of care upon hospital discharge. Solidifying value by quality outcomes of care with hospital case managers, while increasing commitment for utilizing ProMedica Hospice Services. Completing general inpatient referral to admission process within hospital campus. Completing initial hospice benefit discussion with patients & families for electing ProMedica Hospice. Finishing hospice election documents for patients leaving hospital for outpatient hospice services. Participating in rotational, secondary on-call duties 2-4 nights a month Responsible for increasing hospice and palliative care referrals and completing general inpatient hospice admissions with quality, compassionate and collaborative care. Licensed and in good standing in the state in which he/she will practice. Position Requirements Minimum of two years of experience delivering home health care, IV care or hospice services.
