Full-Time Reading Office Job Details Description This position will be responsible for the coordination and oversight of the medical needs, care, and treatment for people supported by InVision Customized Services.  ESSENTIAL FUNCTIONS: Review and maintain medical history and medical records as it relates to diagnoses, physical health, health prevention, medical appointments and follow-up care Ensure physician recommended treatment is received in accordance with regulation and best practices Provide support, education, and training to staff, people supported, and their designees on medical diagnoses, prescribed medication, medication management, dietary guidelines and restrictions, and mobility or positioning Ensure the development of standardized and individualized protocols for diagnosis known to effect the IDD population including but not limited to the fatal four diagnoses of seizures, aspiration, dehydration, and constipation Observe, evaluate, and prompt appropriate medical intervention as needed Participate in team meetings and in health-related goal development, implementation, and outcome reporting Perform basic assessment of physical and mental health needs and obtain vital signs as needed Accompany people supported and their staff to medical appointments as needed Coordinate with external medical professionals or team members as needed Advocate for the needs and rights of the people supported when hospitalized or in treatment, as needed Serve as an on-call nurse, as needed Perform other duties as assigned Core Competencies ·        Leads by Example: Models the values and philosophy of the organization, is decisive, fair and adaptable. o   Plans strategically to create growth, improve financial performance and gain competitive advantages o   Is accountable for the department and oneself o   Demonstrates behavior and work style that are consistent with InVision Human Services values ·        Manages Performance: Provides the necessary tools to cultivate the capabilities of others, and to convey confidence in those capabilities by delegating responsibility, promoting autonomy and fostering accountability. o   Sets clear, challenging goals and expectations o   Provides resources necessary to successfully achieve goals o   Recognizes, celebrates and rewards accomplishments ·        Promotes Quality: Ensures the ongoing commitment to achieving and maintaining quality in all areas of responsibility. QUALIFICATIONS  To perform this job successfully, an individual must be able to perform each essential function satisfactorily. EDUCATION and/or EXPERIENCE   Current PA Licensed Practical Nurse Experience in Electronic Health Records (EHR) Other Requirements: Support the organization’s mission along with sensitivity to cultural and workplace harmony. Ability to maintain confidential information within HIPAA guidelines and organizational policies. Planning and time management and strong organizational skills. Valid PA Driver’s License Management of work processes along with strong computer skills for documentation purposes. Excellent written and verbal communication skills. Thrives in autonomy, independence and graduated levels of responsibility. Act 34 clearance and FBI background check, if applicable  PHYSICAL DEMANDS  Ability to lift and carry up to ten (10) pounds of files. May do some pushing, pulling, standing and walking (1-3 hours) Qualifications
