Employment Type:Full timeShift:Day ShiftDescription:An individual who is responsible for the delivery of varied social work services to Palliative Care patients and their families. The purpose of this position is to provide initial psychosocial assessment, ongoing psychosocial counseling, interdisciplinary care planning, bereavement services, and community education outreach. The Social Worker is an integral part of the Palliative Care Team.Position Requirements:Licensure/Certification/Registration:  Current Georgia’s driver’s license with excellent driving record and automobile insurance as required by law. Georgia Social Work licensure required. LMSWEducation:  Master of Social Work degree from school accredited by the Council on Social Work Education.Experience:  A demonstrated ability in casework, counseling and group work, including two (2) years of clinical work experience preferably in either a health care setting or hospice.Special Qualifications:  High degree of initiative, judgment, and independence in providing social work services and counseling to patient/families while working with the Palliative Care Team.  Able to provide information about psycho-social aspects of patient/family situations, develop appropriate plans of care and interventions.  Coordination of counseling services and community services for patient and family.  Plan and implement bereavement services, counseling and programs. Our Commitment to Diversity and Inclusion Trinity Health is a family of 115,000 colleagues and nearly 26,000 physicians and clinicians across 25 states.
