Duties
Major duties include, but are not limited to the following:
* Responsible for a case load in a community setting involving screening and assessing the psychosocial function and needs of individuals and families from various socioeconomic, cultural, ethnic, educational and other diversified backgrounds and can formulate and implement a treatment plan, identifying the patients problems, strength, needs, abilities, preferences, weaknesses, coping skills and assistance needed, in collaboration with the patient family and interdisciplinary treatment team.
* Requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques.
* Develops psychosocial plans of care which may include individual/family counseling, long-term and advance care planning, non-pharmacological pain interventions, stress management, caregiver respite, psycho-educational programs, as well as grief and bereavement counseling.
* Maximizes VA and community resources/services, such as housing, in-home assistance and financial support.
* Strong ability to independently organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).
* Active interdisciplinary treatment team member who participants in the developing and implementing of a treatment plan by identifying the patient's problems strengths, weaknesses, coping skills, and assistance needed.
* Attends relevant meetings, committees and is responsible for implementation of performance improvement projects with formal evaluation and outcome results.
* The incumbent will utilize a government vehicle to travel within the facility/CBOC coverage area to conduct home visits.
Work Schedule: Monday to Friday, 8:00 AM to 4:30 PM

Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited.
