Job Summary Assigns diagnostic/International Classification of Diseases (ICD-10-CM) codes and Current Procedure Terminology (CPT) codes according to outpatient coding guidelines creating APC assignments for outpatient hospital discharges, emergency room discharges, and ancillary services. # Job Responsibilities Responsible for timely review of patient records in order to identify an appropriate selection of codes that will accurately reflect the reason for admission, extent of care received, and level of severity of illness. Assigns and sequences International Classification of Diseases (ICD-10-CM) diagnosis and Current Procedure Terminology (CPT) codes correctly. Queries providers when documentation is not straightforward, inadequate, or unclear for coding purposes. Keeps abreast of coding guidelines and reimbursement reporting requirements with continuing education activities and reviewing pertinent literature. Brings identified concerns to department manager for resolution. Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association and adheres to official coding guidelines. Qualifications High school diploma, General Equivalency Diploma (GED) or higher required upon hire No experience required RHIT � Registered Health Information Technician through American Health Information Management Associate (AHIMA) required upon hire OR RHIA � Registered Health Information Administrator through American Health Information Management Associate (AHIMA) required upon hire OR CCS � Certified Coding Specialist through American Health Information Management Associate (AHIMA) required upon hire OR CPC - Certified Professional Coder through American Academy of Professional Coders (AAPC) required upon hire Skills and Attributes Requires critical thinking skills and decisive judgment. Works under general supervision. Must be able to work in a stressful environment and take appropriate action. Requires a strong understanding of the specialized concepts, practices, and procedures in the area of specialization.
