**Description:**
I. Position Summary: Assists in the referral and case-management process services by serving as a resource person, providing information about services and assisting the referral source in identifying care needs. Conveys current, accurate information to consumers, payers and other health care providers regarding health care system services. Coordinates the care and service of the patient population across the continuum. Promotes effective utilization of health care resources.
II. Position Requirements: Must be able to analyze problems and find acceptable solutions, to plan and organize work, work with others, and to exercise independent judgment. Must have effective interpersonal skills to obtain and interpret information appropriate to patients' needs and age as required for coordination of services.
A Licensure/Certification/Registration: RN with current Georgia License or an LPN with a current Georgia license and at least 2 years experience in case management. Current Georgia driver's license with an excellent driving record and insurance required by law. Active membership in professional organizations and professional liability is recommended.
B Education: Must be a graduate of an accredited school of nursing. B.S.N. recommended.
C Experience: Three years in nursing and at least one year experience in medical, surgical, geriatric, coronary care, or home care is preferable. LPN candidates must also have 2 years experience in case management. Basic knowledge of computer software and hardware is preferred.
D Special Qualifications: Position requires a person who projects a professional appearance and conducts his/herself in a businesslike manner at all times. Must be mentally alert, conscientious, tactful and have a good memory. An even tempered personality is a desired characteristic. Must be able to work independently and efficiently. Must be ale to recognize and deal with priorities.
