NURSE NAVIGATOR Nursing Hours: 0800-1700 Days - Full-time Paygrade/Wages/Salary Info: OH 2013-76 - 40273A
Job Summary Serves as a patient advocate, liaison, and advisor/educator to assist patients with navigating the continuum of care from detection of disease through acute care treatment. Provides individualized assistance to patients, families, and caregivers to help overcome barriers and facilitate timely access to quality medical and psychosocial care. Enhances the patient#s quality of life, sense of autonomy, and self-determination for managing own health. # Job Responsibilities Rounds on and reviews assigned patients regularly and evaluates patient progress with plan of care. Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution. Provides support, counseling, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making. Serves as a liaison between patient and clinical staff, physicians, managed care companies, and external community resources to coordinate/maximize resources and identify/resolve barriers to the plan of care. Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources. Intervenes as appropriate to advocate for patient and family. Provides educational resources, support, and coordination to assist patients in securing appointments. Qualifications Associate#s degree or higher or Diploma in Nursing required upon hire AND A minimum of 4 years# relevant experience required RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire Skills and Attributes Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action. Department specific competencies for positions are detailed in the unit/department scope of service.
