Join our team today as a Clinical Liaison to positively impact the experiences of our patients and families.Position SummaryAs a Hospice Clinical Liaison, under the direction of the Hospice Area Marker Executive, you will be responsible for assisting referral sources in identifying patients with care needs that match the services provided by hospice. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.Principal AccountabilitiesSupports Program and corporate policies, goals, and objectivesPromotes a positive working relationship between Program and corporate personnel.Attends and participates in staff meetings, educational programs and community events, as requested.Promotes hospice philosophy.Communicates identified needs and potential solutions to Supervisor.Represents Compassus and its services in a competent, professional manner consistent with the organization’s service and teamwork culture.Explains hospice services and benefits to patients, family members, and significant others in a manner that facilitates an accurate and positive view of hospice services.Obtains signatures on all required forms, provides copies to patient/responsible party.Ensures all forms are completed, dated, and signed, and turns in to the Admission Coordinator.Ensures all available medical records are turned into the Admission CoordinatorEstablishes and maintains significant professional relationships.Works closely with community, hospital, and facility education representatives to meet growth and development targets and maintain market acceptance and allegiance.If RN, additional responsibility includes:Facilitates the admission process to and from hospital and long-term care based referral sources.Assesses referred patient’s clinical needs and disease status and confers with patient’s attending physician and hospice physician regarding assessment to establish eligibility.Facilitates patient discharge from hospital to home by establishing initial needs and coordinating transportation, DME, medication needs, supplies and admission by home hospice nurse.Oversees GIP patient admission process based on identified symptom management needs for that level of care.Works closely with facilities and hospitals to identify, transition, and admit patients appropriately for general inpatient services.Provides education to hospitals and facilities on the purpose, process, and goal of general inpatient care.If LPN/LVN (California Only), additional responsibility includes:Facilitates the admission process to and from hospital and long-term care based referral sources.Assesses referred patient’s clinical needs and disease status and confers with patient’s attending physician and hospice physician regarding assessment to establish eligibility.Facilitates patient discharge from hospital to home by establishing initial needs and coordinating transportation, DME, medication needs, supplies and admission by home hospice nurse.Provides education to hospitals and facilities on the purpose, process, and goal of general inpatient care.Performs other duties as assigned.Knowledge and SkillsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Hospital and/or long-term care clinical experience highly preferred. Two to three years’ experience required.Registered NurseCurrently licensed as a Registered Nurse in the state of employment.Licensed Practical Nurse/Licensed Vocational Nurse (California Only)Currently licensed as a Licensed Practical Nurse/Licensed Vocational Nurse in the state of employment.Social Worker (California Only)(A) Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education (preferred); or(B) Has a baccalaureate degree in social work from an institution accredited by the Council on Social WorkEducation; or a baccalaureate degree in psychology, sociology, or other field related to social work; and has 1 year of social work experience in a healthcare setting and is supervised by an MSW; or(C) Has a baccalaureate degree from a school of social work accredited by the Council on Social Work Education, is employed by the Hospice Compassus before December 2, 2008, and is not required to be supervised by an MSW.Mathematical SkillsAbility to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percentage.Language SkillsAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications.Reasoning AbilityAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form.Computer SkillsProficiency in Microsoft Office Suite.Other QualificationsStrong organizational and time management skills. Must be able to work a flexible schedule to include nights and weekends. Must be able to travel.Certifications, Licenses and RegistrationsA valid driver’s license and auto liability insurance.
