Job Description
*Position Summary: *BHCC Discharge Planner - Advantage Behavioral HealthBehavioral Intervention Specialist working in a Behavioral Health Crisis Center that provides short-term stabilization for individuals experiencing mental health and substance use crises. This Behavioral Intervention Specialist assists clients in meeting basic needs in regards to community resources upon time of discharge while collaborating with client and/or family/agencies to create recovery-focused discharge plans. This Behavioral Intervention Specialist collaborates with other members of the treatment team, including psychiatrists, other prescribers and clinicians, to coordinate care.*Duties and Responsibilities: *BHCC Discharge Planner - Advantage Behavioral Health* Meet with client to follow up with discharge plans to assure that everything is correct and nothing has changed.* Obtain Releases of Information and gather information from hospitals, treatment providers, and other agencies who are serving/have served the client.* Collaborating with clients, their family and significant others and other members of the treatment team to create recovery-focused discharge plans.* With signed permission from the client/guardian, communicate with other agencies to coordinate services, including by not limited to Department of Family and Children Services, Probation, Courts, Schools, etc.* Refer for other services, as indicated.* Complete all documentation and processes necessary to discharge clients from treatment when appropriate.* Arrange appropriate transportation for discharging clients.* Attend required staff meetings and trainings.* Facilitate completion of client satisfaction surveys at end of BHCC stay.* Other duties as assigned to support a smooth discharge and transition process*DISCHARGE ASSISTANT DAILY ROLES - *BHCC Discharge Planner - Advantage Behavioral Health* Staff with clinicians for any updates on clients.* Follow up with clients to assure that discharge plan has not changed from meeting with discharge planner.* Complete documentation in the electronic health record for every interaction related to client’s care.* Reach out to facilities to determine bed availability when appropriate for client’s needs.* Communicate with medical records department to request that medical records be sent to aftercare programs as needed.* Request follow up appointments for all outpatient services, including external services.* Request Community Transition Planning meetings with outpatient providers for individuals re-admitted to the CSU within 30 days* Request TRIP$ or Medicaid transport when appropriate and print off scheduled TRIP$ summary daily.*Minimum Qualifications**: *BHCC Discharge Planner - Advantage Behavioral Health*BIS 1*: High school diploma or GED AND One year of experience in a social services related position OR Two years at an accredited college or university with at least 15 credit hours in a social services related field.*BIS 2*: Bachelors degree in a social services field from an accredited college or university.*Certifications, Licenses, Registrations*:Valid GA Drivers LicenseJob Type: Full-time
