SUMMARY OF POSITION:
The Social Worker provides emotional support and linkage to community resources for patients and families, based on an ongoing Social Worker Assessment of Need. The Social Worker plans, implements, and evaluates care through systematic assessment of psychosocial needs and participates in community support groups and bereavement care.

QUALIFICATIONS:
Professional:
- Master’s of Social Work Degree required.
- Must be a Licensed Social Worker.
- At least one year experience in healthcare setting.
- Knowledge of current medical terminology.
- Ability to comprehend applicable federal, state and local regulations and policies, procedures and programs.
- Hospice experience with knowledge of social services regulations preferred.
- Desires to work as part of a team and willing to promote the principles, views and mission of Hospice and Palliative Care of Western Kentucky
Personal:

- Must abide by HWKY Code of Ethics, Core Values and Anti-Values.
- Must demonstrate effective communication skills with team members, clients, and family members.
- Must carry the Kentucky state minimum vehicle liability insurance.
- Must have the ability to work independent of direct on-site supervision.
- Demonstrates leadership and teaching ability.
- Must have current driver’s license, and arrange travel to and from work.
- Must be Covid Vaccinated or receive an applicable exemption approved by Hospice.
Physical Job:
- Must be able to access patient homes independently.
- Must demonstrate proper use of personal protective equipment and observe universal precautions.
- Is willing to work beyond normal working hours and on weekends and holidays when necessary.
Job Type: Part-time
Pay: From $19.00 per hour
Schedule:
• 8 hour shift
