Overview
Bluestone Physician Services is a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive, and includes all members of the care team, patients and their families.  Join us in our team-based care as a Nurse Practitioner or Physician Assistant as we deliver on our promise to tailor primary care to geriatric patients within Assisted Living and Memory Care communities in and around Tampa , FL.
Position Description
• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200+ patients (approximately 12-16 patients per/day)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as needed
Qualifications
Minimum Qualifications
• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC or NCCPA
• Licensed or ability to obtain licensure through the Florida Board of Medical Practice Licensure
• Ability to obtain and /or maintain DEA certification for the State of Florida
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid driver’s license and minimum state-required liability auto insurance; local travel is required
• Must have received or be willing to receive COVID-19 vaccination by date of hire; accommodations will be considered
Preferred Qualifications
• 2-4 years clinical experience
