Perform outreach related to Population Health prevention and wellness initiatives, campaigns and engagement. Responsible for coordination of outreach activities via marketing, mailing and in-person in our regions. Responsible for helping move programming efforts forward working across departments to ensure contract compliance on NC Medicaid requirements and reporting.Role and Responsibilities:Promote and protect public health in the community through education about prevention and wellness resources and programs that promote both physical and behavioral healthMaintain data of outreach events, activities, and contacts along with the people reachedMember navigation and outreach for health and prevention engagementOutreach, coordinate, and conduct health initiative events in the communityEnsure HIPPA compliance in all scope of workPlan and execute outreach and education activities both in house and in the communityCoordinate the necessary materials and staff for the eventsCollaborate with Population Health, Care Management and Marketing and Communications for direction with all activitiesBuild new partnerships with community-based organizationsAttend meetings pertaining to health topics such as prenatal care, diabetes management, etc.Conduct focus groups with member population and community when necessaryGive presentations and speak in a public setting as neededAssist with marketing tasks such as creating and/or editing flyers, maintaining social media (Facebook, Twitter, and Instagram)Actively participates in agency performance improvement initiatives and quality assurance activitiesSubmits agency forms and documentation in a timely mannerAttend on-site/off-site community engagement activities and on-site/off-site clinic events as neededPerform other duties as assigned to support Partners contractual requirement, Mission, Vision and ValuesKnowledge, Skills and Abilities:Candidate must have a thorough understanding of NC Managed Care Medicaid TransformationEmployee has a demonstrated care and commitment to the specialty populations we serveMust understand and utilize identity/person-first language and identity/person-first perspectivePossess exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt decisions and resolve issuesDemonstrate effective oral and written communication.Establishing and maintaining favorable working relationships with members, families, stakeholders, public officials and Partners teamsProactive customer service, problem solving and conflict resolution skillsStrong time and project management skillsExcellent computer skills; Proficiency with Word, Excel, PowerPoint · Natural problem solver with a bias to actionExperience with advocacy and empowerment of membersIs knowledgeable of and maintains HIPAA standards of privacy and confidentialityKnowledge of the laws governing the treatment of health, mental illness, traumatic brain injury, substance abuse and intellectual/developmental disabilities as well as the resources available in the community for treatmentAttention to detailExcellent computer skills and experience mastering new software and platformsAbility to complete tasks independently, define problems, apply laws, policies and procedures to agency activities and must use sound judgment in conducting screening, triage and referralAbility to use sound judgment when conducting outreach and be able to determine when it is necessary and appropriate to transfer a member to a Licensed Access to Care ClinicianAbility to communicate effectively orally and in writing, have good keyboarding skills and be able to converse while entering data into the electronic medical record and evaluating the member’s needAbility to provide technical assistance to both members/recipients and providers or other stakeholdersEducation/Experience Required:Associate degree in health or related field required; BS is preferred. Must have ability to travel. Minimum 2 years’ experience working with public healthcare and related services with families, special populations, and community stakeholders OR an equivalent of (6) years of “lived experience” OR a combination of the two; 2 years of experience may be substituted for every year of education required. Must reside in North Carolina.Education/Experience Preferred:Bachelor’s degree with at least two years of progressive experience in the field of public health, mental health, intellectual/developmental disabilities, traumatic brain injury or substance use disorder is preferred. Bilingual preferred.
