CPCHG APP - Cabarrus
Carolinas Palliative Care & Hospice Group (CPCHG), is currently seeking an experienced Advanced Practice Provider (APP) to join their growing hospice team in Cabarrus County, NC.
Position Highlights:
• Work/life balance is prioritized and hours are Monday-Friday 8:30am-5:00pm in an outpatient setting.
• Rotation for night and weekends are every 7 to 8 weeks.
• Join interdisciplinary team (3 Physicians & 7 APP/s) with collegial relationships that foster team-oriented care.
• Serve as attending practitioner for the home care teams, hospice regulatory requirements including face to face visits in the home, symptom and initiation visits and symptom calls during working hours.
• Maintain quality of life for the hospice patients and supporting their families and have a clear understating of the hospice philosophy.
Who you are:
• Graduate from an accredited NP program with master’s degree or doctorate degree in nursing practice. (Acute Care, Adult, Family, or Gerontological concentration)
• Willing and able to receive Advanced Nursing Certification in Hospice and Palliative Care within 18 months of hire.
• Possess Palliative Care and Hospice experience including care of with complex, chronic and/or patients with a serious illness and those at end of life.
• Experience working with an interdisciplinary team
• Board certification in Hospice and Palliative Medicine required within 18 months of hire.
Recent graduates will be considered with a Palliative Care Fellowship.
