Discovery At Home is hiring a Speech Therapist for Hillsborough County including the Westchase and Sun City areas.
In this role you will provide skilled Speech Therapy services including evaluating and developing plan of cares in order to implement specific treatment programs for individual patients based on their needs and in accordance with the principles of the profession of Speech Language Pathology.
Responsibilities:
• Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
• Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
• Assists in development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
• Consults and familiarizes staff regarding the established plan.
• Teaches, supervises, and counsels the family and client in the speech therapy program.
• Records on a timely basis all evaluation data, treatments, and client's response to therapy intervention. Also records quality assurance data.
• Prepares an assessment of the client's condition and periodic evaluations and summaries.
• Records and reports to the physician the client's reaction to treatment and /or any changes in the client's condition.
• Communicates with the physician at least every 30 days or whenever changes occur regarding the client’s status and obtains additional orders, if necessary.
• Communicates with other team members
• Writes clinical/progress notes on the date of the visit and incorporates same in the client's record within 72 hours.
• Participates in case conferences, staff meetings, agency activities and in-service programs as necessary.
• Participates with staff, clients, and physicians in discharge planning activities and completes a Speech Therapy Discharge Summary.
• Understands and adheres to established policies and procedures of agency.
• Participates in Quality Assessment and Performance Improvement activities as assigned.
• Attends all patient care conferences as scheduled.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.
• Obtains a medical history, particularly as it relates to the present condition.
• Evaluates the client, the client’s significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other Agency/community services will be required.
• Assures that the physician’s orders are appropriate and discusses necessary changes.
Qualifications:
• Required- CCC-SLP (Speech Language Pathology)
• Required- Speech Therapy License
• Must have a minimum of one year of experience in a home health agency.
• Must have knowledge of Medicare and Medicaid guidelines.
• Must have a working knowledge of home health care and the principles and techniques of professional nursing and required documentation that pertains to it.
• Must have a valid driver’s license.
• Must have a valid CPR certification
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.
