ORGANIZATIONAL SUMMARY:
The Pediatric Infectious Disease Division is responsible for managing the activities associated with several grants involving prevention and support of youth and adolescents that are or could be impacted with HIV/AIDS.
POSITION SUMMARY:
This position is responsible for providing education support services and counseling to patients, family members, and support groups regarding the care and treatment of children and adolescents living with HIV. Psychosocial support services, mental health screeners, and case management are provided to all patients and families in the program. This position serves as a liaison between patients and the multidisciplinary team to develop education and provide assistance
RESPONSIBILITIES:
Serves as a liaison between the clinic team and families. Responsible for outlining mental health concerns related to the diagnosis. Manages the development, implementations, coordination, and evaluation of HIV education programs for the pediatric and adolescent HIV programs. Provides the developmental assessment tools and curriculum for this patient population. Compiles data for evaluating program performance. Performs needs assessments, evaluate and analyzes individual project and overall program effectiveness; implements appropriate procedures to ensure efficiency and accuracy to fulfill program mission and meet funder's approved deliverable.
Responsible for assisting clients with access and completion of eligibility services. Informs clients of documentation needed prior to appointments. Conducts all follow up calls with clients as a reminder for documentation and eligibility expiration. Maintain social service records, resources, and databases. Assist in case management of exposed babies in Hillsborough County. Assist in identifying alternative funding sources (Ryan White, ADAP)
Responsible for providing input as it related to education and training during pediatric HIV team meetings and clinics. Makes recommendations and facilitates social support for patients and families. Provides educational support services to patients, family members, and support groups regarding the care and treatment of children with infectious diseases or preventive measures to be taken to guard against infection. Identifies and educates youth on accessing transportation for appointments. Assist in the Insurance Services Programs (ISP) enrollment, screen clients for Medicaid.
Serves as a liaison for those seeking financial and legal counseling during the referral process. Collaborates with physicians, nursing coordinators, community based managers, CMS, child life therapist, psychologist, psychiatrists, and chaplain/clergy services to develop medical education and assistance for HIV/AIDS patients. Communicates with community-based agencies to provide wraparound care for patients. Partner with existing USF Health initiatives to broaden scope of psychosocial and behavioral health services to obtain funding for future prevention, evaluation, and treatment effort.
Recommends/implements plan for those areas requiring improvement. Coordinates the yearly developmental evaluations and psychiatric appointments for patients in the program. Responsible for meeting with patient's school personnel. Participates in early intervention plans and follows up on recommendations by psychologist/psychiatrist. Services as a liaison with community, outreach, and testing agencies to bring newly identified youth into care. Designated liaison between clinical team and families.
Develops a database and tracking tools to monitor the success of the program and generate reports to the team and sponsor. This includes but is not limited to electronic medical record entry, analysis and integration between Department of Health EMR (HMS), Florida Shots, and USF EMR (EPIC). Assist with Front Desk training, schedule patient appointments.
Performs other duties as assigned.
POSITION QUALIFICATIONS:
MINIMUM:
Each position assigned to this classification must require specific training at the Bachelor's degree or Master's degree level in Education, Special Education, Psychology, Social Work, or other field appropriate to the program content. Bachelor's degree candidates must have at least two years of applicable experience. The Master's degree may substitute for the required experience. Specific certifications or licensures may be required or preferred for certain program areas.
SPECIAL WORKING CONDITIONS:
Evening and weekend work may be required at times, as well as on-call duty for emergencies.
