The Market Medical Director (MMD) will serve as a key member of the market leadership team. He/she will report directly to the Regional Medical Officer but also have dual reporting to the Market Executive Director. The MMD’s primary responsibility is to provide clinical and operational oversight to the physicians and advanced level practitioners in the market and provide direct and indirect medical care and population health management to patients served by Landmark Health. This role also acts as a leader for the market’s interdisciplinary team accountable for serving the markets patient population.
Primary Responsibilities:
• Direct & Indirect Care
• Provide consultative recommendations of the most complex patients within the population
• Provide clinical leadership, oversee quality and execute clinical operations within the market
• Responsible for providing the means, structure, and environment to most effectively utilize the resources that are available to ensure that all enrolled members have access to appropriate medical care
• Mentor and guide associate medical directors in reviewing reports to improve population health for patients within the market
• Oversee associate medical director chart reviews as well as complete targeted comprehensive chart reviews and ride alongs of providers in the market coordinating with other market physicians in order to ensure high quality care of patients
• Participate in the engagement of high utilizer patients
• Participate in the market on call schedule and provide telephonic on-call coverage that require telephonic triage based on an as needed basis
• Provide real time supervision of Urgentivist Extenders (paramedics or RNs/LPNs/LVNs) as needed
• Accountable for markets performance against key clinical and operational performance metrics
• Remain current in the knowledge of geriatrics, utilization management, quality management, managed care principals and new clinical developments and technologies
• Oversee and participate in the development, documentation, implementation, and interpretation of local medical policies and procedures
• Participate in the continuous assessment and improvement of the quality of care provided to members
• Participate in the resolution of medically related grievances
• Team Performance Management
• Supervise associate medical directors and advanced practitioner supervisors, including monitoring and conducting regular 1:1s, proactively providing performance feedback, reviews and mentorship, overseeing provider education, in-service training, and orientation
• Supervise market pharmacists and behavioral health advanced practitioners in the market who also have a dotted line to leadership in their discipline
• Is the clinical hiring manager for the market and recruits physicians and advanced practice practitioners, ensuring the pursuit of talent with the appropriate mix of clinical skills and behavioral qualities to retain a high performing team
• Oversee provider education, in-service training and provider orientation in the market
• Provide education and share best practices: Landmark Way trainings, additional staff education, and coordinate education sessions with outside agencies
• Assume oversight responsibility for all components of medical management in the market
• Assume responsibility for the administration of all medical activities in the market
• Interdisciplinary Team Leadership
• Collaborate with leaders in outreach, health services, pharmacy, dietary, behavioral health and others to ensure the highest quality medical care at the best cost
• Participate in clinical excellence education to providers in the market
• Participate in weekly clinical leadership team meetings and other senior medical director meetings as necessary
• Participate in zone meetings and promotes equity of work across the zones
• Lead and deliver Landmark Way presentations and delegate this activity to a market associate medical director when unable
• Participate in and lead as necessary in interdisciplinary team meetings, including case reviews that engage the full team, teaching on relevant clinical topics, and training on new protocols and workflows
• Act as a local leader and advocate for the Landmark model with community providers and delivery systems and other community partners or potential business partners
• Health Plan and Community Relations
• Build and enhance community relationships with targeted hospitals, high volume PCPs, community acute care, SNF, LTC and ALF facilities in the market
• Join meetings with Chief Medical Officers of targeted Health Plans as needed
• Serve as an advocate for the team with community providers when escalation is necessary and ensure appropriate resolution
• Strategy & Standardization
• Support the Regional Medical Officer as needed with development of strategy
• Support the Regional Medial Officer with enforcing clinical standardization across the market
• Oversee the standardized application of specific measurements of quality and ensure that all providers consistently meet thresholds
• Support implementation of centrally-led initiatives Other
• Collaborate with shared services on special projects and company-wide initiatives on an as needed basis
• Partner with the other market leaders to create and present at quarterly business reviewsOther duties as assigned by the Regional Medical Officer

Required Qualifications:

• Physician currently licensed to practice without restriction in the state of practice and has met or exceeded all the credentialing requirements of a provider within the provider panel
• Experience in leading teams of physicians and mid-level providers in delivering high quality, cost effective care
• Experience in recruiting and hiring medical talent along with continuous supervision of the medical care delivered
• Experience in coaching members of the health care team to provide excellent care or, if needed, determine deficiencies in care and formulate a plan for improvement
• Current in the knowledge of geriatrics, utilization management, quality management, managed care principals and new clinical developments and technologies
• Ability to analyze staffing needs sufficient to care for the patient panel
• Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety Policy
