Primary Purpose of Position: *
Provide training, health and wellness education and resource information to members and stakeholder agencies of Partners Tailored Plan as directed by operations teams and Population Health Plan. Responsible for helping move programming efforts forward working across departments to ensure contract compliance on NC Medicaid requirements and reporting.

Role and Responsibilities: *
• Assess the health needs of individuals and communities
• Develop programs, materials, and events to teach members about health topics, such as managing existing conditions and lifestyle changes
• Evaluate the effectiveness of programs and educational materials
• Help people find health services or information
• Provide training programs for community health workers or other healthcare providers
• Promote and protect public health in the community through education about prevention and wellness resources and programs that promote both physical and behavioral health
• Maintain data of outreach events, activities, and contacts and manage related reporting with systems analysts
• Member navigation and outreach for health and prevention engagement
• Outreach, coordinate, and conduct health initiative events in the community
• Ensure HIPPA compliance in all scope of work
• Assist in planning, developing, and conducting workshops, conferences, and training opportunities for professional and lay groups)
• Prepare and disseminate news releases and feature stories to the press, radio, social media, and television
• Collect and analyze data to learn about regions and communities and improve health resources and services
• Advocate for improved health resources and policies that promote health and health equity
• Collaborate with internal teams and external partners to meet NC Medicaid contractual requirements
• Follow Marketing & Communications procedures to publish, share and maintain training and education materials across platforms
• Service delivery settings may include but are not limited to Partners’ county coverage region

• Is knowledgeable of and maintains HIPAA standards of privacy and confidentiality
• Knowledge of the laws governing the treatment of health, mental illness, traumatic brain injury, substance abuse and intellectual/developmental disabilities as well as the resources available in the community for treatment
• Possess strong attention to detail
• Excellent computer skills and experience mastering new software and platforms
• Ability to complete tasks independently, define problems, apply laws, policies and procedures to agency activities and must use sound judgment in conducting screening, triage and referral
• Ability to use sound judgment when conducting outreach and be able to determine when it is necessary and appropriate to transfer a member to a Licensed Access to Care Clinician
• Ability to communicate effectively orally and in writing, have good keyboarding skills and be able to converse while entering data into the electronic medical record and evaluating the member’s need
• Ability to provide technical assistance to both members/recipients and providers or other stakeholders
• Must possess strong statistical knowledge, analysis, evaluation, and data entry skills
• Conduct surveys to provide better health outcomes
• Be able to work in a small team environment with a high degree of independence and self- management
• Be a self-starter with excellent follow-through and organizational skills

Education/Experience Required: *
Bachelor’s degree with at least two (2) years of progressive experience in the field of public health, mental health,
intellectual/developmental disabilities, traumatic brain injury or substance use disorder and four (4) years of training/education experience required. Bilingual preferred.

Education/Experience Preferred: *
A Master’s degree in public health education or in public health with a major in public health education; or graduation from a four-year college or university with a major in health education and four years of experience with training/education.
