Primary Purpose of Position: *
The Tailored Care Management (TCM) Screening and Referral performs intermediate administrative work as well other qualified professional duties related to overseeing the coordination and management of care for members and recipients. The TCM Screening and Referral Specialist will assist members and recipients with requests to change Care Managers, Opt-out, or engage/re-engage with Care Management. This position will ensure accurate documentation in TruCare and Alpha+ regarding Care Manager assignment. This position will complete Care Needs Screenings for members who are ineligible for Care Management or have chosen to Opt-Out. This position will monitor the queue of members unengaged in Care Management. This position will assist with securing and monitoring Care Coordination efforts for recipients enrolled in state benefits. This position is a member of the Care Connection Team. This is a mobile position with work done in a variety of locations.

Role and Responsibilities: *
The TCM Screening and Referral Specialist functions may include but are not limited to:
• Communicate with members and recipients via phone to field questions and provide education on the TCM approaches, request to change CM process, and Opt-out process
• Complete Care Needs Screenings for members not eligible or opted out of TCM.
• Connect members and recipients not participating in TCM with transitional care as necessary
• Document all member/recipient related interactions in TruCare.
• Convene key providers (including other entities providing care management) and others to address needs of the individual, through team meetings.
• Be responsive to requests from other entities providing care management to members excluded from Tailored Care Management
• Assist in identification of barriers to case load management and in development of strategies to address said barriers
• Provide QP level care coordination or case management activities to members and recipients as well as their families.
• Coordinate a range of scenarios, which require a wide array of potential responses including, but not limited to: informing, researching, linking, assessing need, reviewing documentation, phone communication, and consultation.
• Perform care coordination to ensure continuity of care and clinical evaluations are performed as needed for appropriate provisions of intensive, crisis services and other levels of care.
• Promote integration of physical and behavioral healthcare for at-risk enrollees including ensuring linkage to a primary care practice and collaboration with the Community Care of North Carolina staff as appropriate.
• Track and report performance measures to supervisor as assigned.
• Provide discharge planning assistance to team, local hospitals and track individuals discharged from state and local hospitals to ensure they follow up with aftercare services and receive needed assistance to prevent further hospitalizations.
• Document accurately, comprehensively, and timely all contacts and other care coordination functions.
• Facilitate the integration of continuous quality improvement in areas supervised.

Knowledge, Skills and Abilities: *
• Knowledge of Tailored Plan, NC Medicaid Direct, and state funded benefits
• Highly skilled at assuring that both long and short-range goals and needs of the individual are addressed and updated, while assuring through monitoring activities that service implementation occurs appropriately
• Exceptional interpersonal and communication skills
• Excellent computer skills including proficiency in Microsoft Office products (Word, Excel, Outlook, and PowerPoint)
• Excellent problem solving, negotiation, arbitration, and conflict resolution skills
• Detail-oriented, able to organize multiple tasks and priorities and effectively manage projects from start to finish
• Ability to make prompt independent decisions based upon relevant facts, to establish rapport and maintain effective working relationships
• Ability to change the focus of his/her activities to meet changing priorities
• A high level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance

Education/Experience Required: *
• Bachelor’s Degree in Human Services and two (2) years of MHSU experience, OR
• Bachelor’s Degree in a non-Human Service field and four (4) years of MHSU experience.
Other requirements:
• Must have ability to travel as needed to perform job duties.
