Compassionate Hospice Admissions Nurse
Seeking RN to provide compassionate and exceptional in home patient assessment and care for those considering or in in need of admission to Trellis Supportive Care services for end of life care

Trellis Supportive Care is a not-for-profit hospice and palliative organization, serving patients and families in a Piedmont/Triad since 1979. Founded by a group of compassionate visionaries and community leaders, Trellis Supportive Care has remained a pace-setter in the region, state, and in the nation and has expanded over the decades to meet the supportive needs of our patients. We have made it our mission to provide compassionate care for individuals living with a life-limiting illness and their families, through quality medical, emotional, spiritual, and social support wherever care is needed. Our specially trained professionals work as a team to help patients live fully and comfortably, with hope — even during life’s most challenging time.
Various Schedules Available
40 hours per week ( Full Time); Mon-Fri 9a - 6p,10a - 7p, OR 12noon - 9p with occasional Saturday rotation (in lieu of a week day that week)
Serving Rowan, Davie & surrounding areas

Responsibilities
• Assessing and evaluating needs of hospice patients and families in their homes
• Determine eligibility for hospice services prior to admission
• Educate the family about services
• Establish appropriate nursing care plans based on needs of patient and family
• Compassionate and exceptional patient care in accordance with the plan of care
• Communicate with physicians and other members of the interdisciplinary team to ensure high-quality care
• Exceptional communication with patient and family to develop and follow the plan of care
Requirements
• Current NC license as a Registered Nurse
• A minimum of two (2) years’ clinical experience preferred
• Excellent nursing assessment and problem-solving skills
• Excellent interpersonal communication skills
• Exceptional customer service
