Alternate Family Living (AFL) Contractor
*This is an opportunity for an independent contractor and is not an employment position.*
This service provides individualized supports to enable persons who possess intellectual developmental disabilities and/or possible mental health challenges to live successfully in a setting of their choice and be an active participant in his/her community. The intended outcome of the service is to increase or maintain the person’s life skills, provide supervision as needed, maximize his/her self-sufficiency, increase self-determination, and ensure the person’s opportunity to have full membership in his/her community.
All supported individuals have their own identity with varying levels of needs and Abound Health thrives on connecting people for lasting successful partnerships. As an AFL provider, you will offer a stable and caring home where individuals can live, thrive and integrate into the community through various means. Depending on the supported individual, he/she may attend a day program, vocational services, be employed, or receive other services in addition to what the AFL will be providing with residential services. All services are provided in a collaborative effort with many people involved in the planning and routine monitoring, to ensure overall success.
Requirements:
• High School Diploma or GED
• Pass a physical
• Pass criminal record and DMV check, healthcare registry and Office of Inspector General
• CPR/First Aid Certification is required. Training is provided.
• Residence will be inspected by both the agency and the PIHP, using a standardized checklist. The home must pass preliminary inspection before a final inspection is scheduled and client scheduled to move in.
• Must be able to lift up to 25 to 50/lbs
