Full Job Description Program Specialist, Grandparents Raising Grandchildren
The Athens Community Council on Aging is looking for a part time Program Specialist to provide case management and support for the kinship families in our Grandparents Raising Grandchildren program.
The Program Specialist works with the Program Coordinator and student interns to connect families with agency and community resources as they progress toward their care plan goals. The Specialist also assists with the coordination of the program in Clarke, Barrow, Jackson, Madison, Oconee, and Walton Counties.

Essential Responsibilities
Case Management
• Provide case management for assigned families including intakes, reassessments, and exiting paperwork, creation and implementation of case plan goals and evaluations of meeting assigned goals, home-setting visits, telephone meetings and family education and support.
• Arrange for other social and casework services to assist in resolving client problems such as those available through county Department of Family and Children Services, Social Security Administration, Action agencies and other appropriate agencies.
• Coordinate and implement client activities such as support groups, education classes, and special events.
• Serve as a liaison and advocate between client, agency, family and community for outreach, general inquiries and education contacts.
• Prepare and file letters, memos, address labels, forms and reports as requested by the Program Coordinator.
• Assist with transportation, nursing, emergency aid, and other services.
Program Operations
• Assist the Program Director in supervising intern students working within the program. This includes supervising the interns’ involvement in the planning and implementation of support groups and activities, assisting with weekly individual and group supervision, and monitoring and evaluating the case notes of interns.
• Assist the Program Director with preparing contract renewal proposals, competitive bids and proposals for current and additional funding sources needed for the program.
• Assist the Program Director with funding source and ACCA paperwork including monthly and annual reports.
• Monitor adherence to approved budget by approving all program expenditures in consultation with the Program Director.
• Assist the Program Director in ensuring that forms and files are maintained accurately and submitted monthly, semi-annual, and annually as required by ACCA Board of Directors, PSSF, and various funding sources.
• Maintain active working relationships and ensure that outreach is being conducted on a regular basis with community agencies and groups through program attendance at health and information fairs, network meetings and serving as a member of appropriate community committees.
• Obtain program supplies as needed and ensure that supplies and equipment are set-up and ready for all program activities.
• Ensure that participant information data for purposes of program reporting and scheduling is gathered within required time frames, based on agency and funding source regulations. Assist with frequent monitoring of record-keeping to ensure compliance with ACCA and regulatory agency requirements.
General Responsibilities
• Completes all appropriate forms including timecards, travel sheets, and purchase orders within deadlines.
• Attends staff meetings, annual agency meeting, training programs, and other appropriate meetings and conferences related to position, and represents the ACCA on coordination and planning bodies in the service area, as requested.
• Assists in the overall fundraising needs of the agency and supports the Director of Development and Communications in the planning and coordination of fundraising and awareness events directly related to GRG. Participates in fundraising committees and attend events.
• Accepts other responsibilities as assigned by GRG Coordinator, VP/Director of Operations, and CEO.
