Full Job Description $1,000.00 Sign On Bonus
SUMMARY OF POSITION:
Delivers quality hospice and palliative nursing care to patients in the inpatient hospice setting and/or the patient’s home. In collaboration with the interdisciplinary team, develops, implements, and coordinates the interdisciplinary plan of care for each patient and family. Performs all aspects of the nursing process including facilitation, communication, assessment, intervention, education, planning and providing comprehensive nursing care for patients and families facing life-limiting illness. Provides direct and indirect patient care and supervises Hospice Aides providing this care. Communicates with physicians, interdisciplinary team members about changes in a patient's clinical, psychological or spiritual needs. Responds quickly and accurately to changes in condition or response to treatment. Acts as a patient/family advocate and an HWKY representative, meeting the patient/family needs throughout the hospice experience. Assists in the development of new staff nurses and the future development of the nursing profession.

QUALIFICATIONS:
Professional:
- Graduate of an accredited School of Nursing.
- Current licensure in the state of Kentucky.
- Knowledge of current trends and techniques in nursing practice.
- At least three years experience in a patient care position in a hospital, nursing home, or related healthcare facility preferred.
- Hospice or palliative care experience preferred.
- Proficient in electronic documentation.
- Demonstrates effective verbal and written communication skills.
- CPR Certification required
- Desires to work as part of a team and willing to represent the principles, views and mission of Hospice and Palliative Care of Western Kentucky.
Personal:
- Personal qualities of empathy, responsibility and dependability.
- Must sign and abide by HWKY Code of Ethics, Core Values and Anti-Values.
- Must demonstrate effective communication skills with team members, clients, and family members.
- Must carry the Kentucky state minimum vehicle liability insurance.
- Must have the ability to work independent of direct on-site supervision.
- Demonstrates leadership and teaching ability.
- Must be Covid Vaccinated or receive an applicable exemption approved by Hospice.
Physical Job:
- Must have a current driver’s license, and arrange travel to and from work.
- Must demonstrate proper use of personal protective equipment and observe universal precautions.
- Is willing to work beyond normal working hours and on weekends and holidays when necessary.
