Job Summary:
Provides In-Reach and Transition Coordination to Individuals who meet the SMI criteria who reside in or at risk of re-entry into a Personal Care Home.In Reach includes providing information on community-based service options and housing alternatives.Responsible for maintaining information on the ISA Tracking tool, entering, updating, and generating reports in a timely manner as stated in the State General Fund Contract.
Highlights:
Determines the individual meets the definition of an Adult meeting criteria for SMI designation
Administers Level of Care Utilization System (LOCUS) and documentation
Coordinates Transition Planning
Works with Case Management and Housing Coordinator to enable clients to move into community with needed supports and services
Minimum Qualifications:
Bachelor's Degree in Human Services field. Master's degree preferred.
One (1) year experience in case management or working with the severely mentally ill population after Bachelor's degree is completed.
