Job Summary:
The MPW Assessor/Waiver CM is responsible for conducting initial MPW Assessments, and Reassessments for outside CM agencies per the State Contract for our 7 county coverage area. MPW Assessor/Waiver CM also facilitates coordination, communication and collaboration on behalf of waiver participants in order to archive goals and maximize outcomes for the participants. they monitor the delivery of services to the participants at various service sites.
Minimum Qualifications:
Bachelor's Degree
1 year experience working with individuals with intellectual and/or developmental disabilitie
Location ID - Owensboro
Relocation is not provided and travel is regularly required
