We are looking for full-time Social Worker to be a part of our incredible team and who have the same vision.
Specific duties for the MDS role includes :
• Primary role is to conduct and coordinate the Resident Assessment Instrument (RAI) that is the Minimum Data Set (MDS).
• Conduct Care Assessments (CAAs) and Care Plans in accordance with State and Federal requirements.
• The MDS nurse participates as part of the facilities Interdisciplinary Team (IDT) to manage patients care.
• Other nursing duties as apply.
