DISCHARGE PLANNER I

The Discharge Planner I provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting
Responsibilities:
• Possesses a clear understanding of discharge and transition planning and linkage with community resources.
• Oversees legal status issues in accordance with the Baker Act regulations.
• Provides support as an active member of the treatment team.
• May be required to travel.
• Will perform other duties as assigned by the supervisor.

Certifications and Licensures:
• Required: BLS (Basic Life Support)
Education:
• Required: Bachelor’s degree in Counseling
• Or: Bachelor’s degree in Health Education
• Or: Bachelor’s degree in Related Field
• Or: Bachelor’s degree in Social Work
• Or Bachelor’s degree in Human Services
• Or Bachelor’s degree in Psychology
• Or Bachelor’s degree in Nursing
• Preferred: Master’s degree in Human Services
Experience:
• Required: 1 year in Behavioral Health
• Required: 2 years of Child and Adolescents (SIPP Only)
• Required: 2 years (Reference Summary for details)
• Preferred: 6 months in Acute Care
Specific Skills:
