Overview:
The Branch Director must be a licensed a Registered Nurse, Physical Therapist, Speech-Language Pathologist, Occupational Therapist, or Social Worker. Provides oversight of all patient care services and personnel and is responsible for the direction, coordination, and supervision of services. Ensures quality of patient care in the home. Coordinates care with the interdisciplinary team, patient, family, and referring agency.

Responsibilities:
• Coordinates patient care. Coordinates and integrates care provided by Agency staff, including contractual providers.
• Complete workflow tasks and other assignments to the BD Responsible Position in the Agency’s electronic medical record.
• Ensures communication with all physicians involved in a patient’s plan of care and other health care practitioners (as appropriate).
• Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
• Coordinates referrals.
• Participates in activities relevant to professional services including development of qualifications. Recommends resources needed to provide care. Makes patient and personnel assignments.
• Assures that patient needs are continually assessed. Assures reassessment of the patient’s needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient’s condition, at the physician’s request, and after hospital discharge.
• Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient and caregiver education and patient, caregiver, and family counseling. Prepares clinical notes as appropriate.
• Assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations.
• Collaborates on development and implementation of policies and procedures which guide and support the provision of care. Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient care.
• Monitors and participates in the QAPI Program and assures appropriate corrective measures are performed.
• Promotes compliance with all fiscal intermediary and/or other third-party payors, through education, coaching, and other assistance as necessary.
• Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
• Meets mandatory continuing education requirements of the Agency and licensing board.
• Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
• Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Participates in Agency-sponsored in-service training.
• Ensures the care offered by the Agency, including any branches, is consistently available to all patients.
Physical, Mental, Miscellaneous Demands
• Prolonged sitting required, with the ability to operate office equipment which may include heavy lifting, bending, and standing.
• Requires the ability to handle stressful situations in a calm and courteous manner at all times. If required to make patient visits, prolonged standing and walking required, with the ability to lift up to 50 lbs. and move patients.
• Requires working under some stressful conditions to meet deadlines and Agency needs.
• Must have and maintain valid Texas Driver’s License, and proof of current valid auto liability insurance.
• Travel to various geographic locations required. Some overnight stays required.
• Requires current CPR certificate, and the ability to maintain a current CPR certificate.
• Requires ability to use up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and/or up to 10 pounds continuously to move objects
• Work deals mostly with areas such as preparing and reading data and figures, records, reports, visual inspection involving small details. Although important, depth perception and field of vision (peripheral) are not as critical as the ability to distinguish small details and markings very near to the observer.
• Requires ability to grasp, pull, push, carry or otherwise manipulate objects; ability to perform tasks requiring action of muscles or groups of muscles and foot and/or hand-eye coordination.
• Requires ability to communicate clearly and make self-understood effectively in face-to-face interactions; articulate with accuracy to communicate using phone.
• Requires ability to hear and receive verbal instructions correctly, answer phones, communicate professionally with people in situations with some background noise.
• Requires ability to concentrate on fine detail with frequent interruption; ability to focus attention on tasks for 10-20 minutes at a time on a continuous basis, 20-60 minutes on occasion.
• Requires ability to understand and relate to specific ideas one at a time and to the concepts behind specific ideas; ability to remember verbal and written tasks/assignments from a few hours to several day periods.
• Position requires sitting approximately 60% of time standing/stooping/bending/ climbing approximately 20% of time; and walking approximately 20% of time.
• Excellent computer software skills necessary to produce accurate documents and materials required.
• Working knowledge of MS Office, Excel or similar software required. Proficient knowledge preferred.
• Requires ability to work effectively under minimal supervision; exercise excellent, sound professional judgment and maintain confidentiality.
Qualifications:
Education : Graduate of an accredited school for the discipline held.
Licensure : Current state license at a Registered Nursing, Therapist, or Social Worker. Has current state Driver’s License.
Experience : Minimum of 2 years of homecare experience and 1 year of supervisory experience is required.
Skills : Clinical skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability to communicate effectively. Demonstrates proven decision-making skills. Excellent interpersonal skills and ability to communicate effectively. If required to make visits, needs proof of current CPR.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental/Working Conditions: Performs duties in an office environment during Agency operating hours promoting efficient functioning and coordination of Agency activities to ensure the highest level of professional patient care. If required to make patient visits, may work in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; the ability to work a flexible schedule; the ability to travel locally; some exposure to unpleasant weather; PRN emergency call. Branch Director or Alternate must be available at all times either in person or via telecommunications.
