Responsibilities:
1. Gather information related to the patient and family physical, psychological, social and spiritual needs based on the interdisciplinary plan of care
a. Collaborate with Hospice Nurse-RN, patient and family, physician and interdisciplinary group (IDG) to coordinate care
2. Initiate and maintain communication with Hospice Nurse-RN, IDG, Team Director (TD), Director-Clinical Operations (DCO), Administrator on Call (AOC) and IDG and other agencies to coordinate optimal care and use of resources for the patient and family
a. Especially, in the event of unusual or potentially problematic patient and family issues
3. Document according to protocol and maintain up-to-date patient records
a. Ensure plan of care problems, interventions, actions and goals reflect baseline, current, and ongoing status, and changes as they occur
4. Participate in Continuous Care, Respite Care or On-Call assignment/rotation, as directed
a. May conduct Information, Sign-On and Consent visits, as assigned
5. Provide education to patient and caregiver(s) as needed, based on knowledge needs and learning assessment, patient and family condition, in keeping with plan of care
6. Attend IDG meetings and other patient conferences, as necessary
7. Participate in orientation and ongoing education programs, on committees and special projects and other related duties as assigned
8. In collaboration with the AOC, the IDG and the cAll Center staff
a. May perform routine visits
b. Coordinate schedule and care activities with the Hospice Nurse-RN, IDG, TD, DOC or AOC and other agencies as necessary
c. May participate in IDG Meetings, facility patient conferences, and other patient and family meetings in coordination with the IDG and/or the TD
9. If serving in an On-Call capacity, the employee is free to spend their time as they choose, until they become engaged in work
10. Performs other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.) Employees should reference the position specific job description for additional details on the essential functions of the clinical discipline
11. Other duties as assigned
Qualifications:
1. Associate degree, preferred
2. Licensed as a Licensed Practical Nurse (LPN) in state(s) practicing
3. Current Cardiopulmonary Resuscitation (CPR) certification, preferred
a. Direct Patient Care in Georgia, required
4. Previous experience in hospice, home health, general medical surgical or patient population specific (i.e., geriatrics, pediatric, etc.) nursing
5. Must have reliable automobile, valid drive's license and the minimum state required liability auto insurance
6. Excellent written and verbal communication and organizational skills
7. Demonstrated familiarity and use of agency and state(s) policies and protocols and federal regulations pertaining to provision of hospice and nursing services
8. Working knowledge of computer programs, especially Microsoft Office Suite products and EMR systems
9. Ability to work as part of an interdisciplinary team
10. Meet the regulations and requirements of the State in which the program provides services
