Social Services Specialist I - Care Coordination
The Social Service Specialist I is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, other practitioners, caregivers, and the community. Provides coordination, referral and interdisciplinary discharge planning. Knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs. Possesses a clear understanding of discharge and transition planning and linkage with community resources. Demonstrates skills in planning, organizing, and managing multiple functions. Required Experience includes 6 months Internship or 6 months Social Work in an acute care setting.
This position is PRN "as-needed" with a variable schedule 8:00 AM - 4:30 PM at St. Joseph's Hospital South. Strongly prefer candidates enrolled in Master's of Social Work, Sociology, or related field.
