Under the: direction of the Director of Nursing Services, the MDS Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

MDS CARE PLAN COORDINATOR QUALIFICATIONS:
• FL Nursing License ( Registered Nurse RN ) Required
• Organizational Skills/Ability to Multitask
• Knowledge of PDPM/Clinical Concepts
• Ability to work with facility nursing management to identify and fix care documentation notes
