Iris Healthcare (powered by Aledade) is looking for full-time Outreach Specialists to join our diverse growing remote team. The ideal candidate should be self-motivated, compassionate, and have a strong background in motivational interviewing and patient outreach. In this role, you will be performing outreach and education efforts via phone and to enroll members in our Advance care planning (ACP) program. You must be able to meet rigorous contact and conversion goals, prioritize performance metrics and balance your daily workload. In addition, the ideal candidate will have excellent verbal communication and phone skills, combined with a background in active listening, motivational interviewing or similar.
This is a completely remote full-time permanent position . This position offers the convenience of working from home. The ideal candidate will have the ability to work Monday-Friday from 9 am-5 pm within your own time zone. Being on camera for video meetings is required.  $17-19/hour pay rate.
Iris Healthcare is an Austin-based company that focuses on empowering members through advance care planning. Advance care planning (ACP) is defined as a process that supports individuals in understanding and sharing their values, goals and preferences for future medical care. These care decisions are then formally documented within an Advance Directive; a forward-thinking legal document that empowers individuals, their loved ones and their care teams to make the best advance care planning decisions moving forward.
Responsibilities: • Handles outbound calls to patients to setup meetings associate with Advance care planning
• Apply motivational interviewing tactics to break down psychosocial barriers
• Document patient outreach and outcome in the system
• Identify outreach trends and communicate those  trends to the team
• Share creative ideas for improving workflows
• Maintain confidentiality of sensitive patient information
• Maintain a good collaborative working relationship with your designated market team & leadership team
Requirements: • 2+ years of experience in healthcare preferable within client management, enrollment, and/or patient outreach.
• Experience in patient outreach, case management or client management, and documentation review
• Bilingual-Spanish preferred
• Compassionate, mission-driven sales or outreach experience
• Advanced Care Planning knowledge or familiarity with end of life care a plus
• Be client-focused, including the ability to put clients at ease and maintain composure while addressing stressful and/or challenging healthcare topics.
• Demonstrate successful results and familiarity in previous work meeting company metrics for outreach, delivering on company and individual conversion goals, and consistently delivering high customer satisfaction rates.
• Desire to make large volumes of outbound phone calls per day to new members while maintaining the confidentiality of sensitive client information.
• Demonstrated ability to work successfully in a remote working environment
• Background in motivational interviewing (MI) or related area. Must have access to strong and reliable internet
