ORGANIZATIONAL SUMMARY:
The Department of Pediatrics at the USF Health is part of The University of South Florida System is a high-impact, global research system dedicated to student success. The USF System includes three institutions: USF; USF St. Petersburg; and USF Sarasota-Manatee.
POSITION SUMMARY:
The Human Services Practitioner/ Social Worker is responsible for providing social work interventions for Pediatric patients.
RESPONSIBILITIES:
Performs initial and ongoing psychosocial assessment and counseling to psychosocial assessments and counseling for all Cystic Fibrosis patients as well as pulmonary patients referred by physicians/providers.
Coordinates all day-to-day referrals, intake activities,
-Mental Health coordinator for Pediatric Cystic Fibrosis patients
-Transition Coordination to Adult CF care programs
Provide education and support to patients and their families in relation to CF diagnosis
Maintain a resource base and refer patients to community resources -Compiles information and prepares monthly report for CMS 'deliverables' to be sent to FL Department of Health Regional CMS Nursing Director. Liaison with Department of Children and Families programs and other Community agencies -Participate in patient staffing and care conferences/disaster planning/care coordination
Participates in training and other educational activities including participation in Annual Cystic Fibrosis Conference of North America.
Performs activities such as home visits, judicial appearance in Family Court as required.
Field Instructor for USF Graduate School of Social Work (MSW program) 2%
Advocacy Program rotation (monthly) for USF Medical Residency program.
Other duties as assigned.
POSITION QUALIFICATIONS:
MINIMUM:
Each position assigned to this classification must require specific training at the Bachelor's degree or Master's degree level in Education, Special Education, Psychology, Social Work, or other field appropriate to the program content. Bachelor's degree candidates must have at least two years of applicable experience. The Master's degree may substitute for the required experience. Specific certifications or licensures may be required or preferred for certain program areas.
PREFERRED:
Master's degree preferred in public health, social work, special education or any related clinical discipline.
SPECIAL WORKING CONDITIONS:
Evening and weekend work may be required at times, as well as on-call duty for emergencies.
