The role you’ll contribute:
The Registered Nurse (RN) Readmission Coordinator, in partnership with patients, caregivers, providers, nursing and the multidisciplinary healthcare team, utilizes professional skills to reduce unnecessary readmissions. Coaches patients and families in disease self-management and establishes plans for safe and effective transfers in the movement of patients across the continuum of care. Promotes and evaluates the effective utilization of resources using current clinical knowledge, awareness of community services to achieve optimal clinical and resource outcomes.
Patients will be identified by various methods including collaborating with the Transition of Care Team members and Providers. Transitional management will be provided to the patients who have the highest risk for readmission and strategies will be appropriate to that level.
The value you’ll bring to the team
• Collaborates with Case Management and Care360 Teams to address discharge needs including Durable Medical Equipment (DME), Home Health Care (HHC) services, Rehab/Skilled Nursing Facility (SNF) placement, and follow-up appointments with Primary Care Physician (PCP) and specialists
• When appropriate, collaborates with Respiratory Therapy and Cardiopulmonary Rehab Teams to assess a need in post-discharge Rehab
• When appropriate, distributes Heart Failure Kits to patients currently hospitalized with Heart Failure Diagnosis and log patient information to the HF Kit Distribution List located on Microsoft Teams
• Collaborates with multidisciplinary team including Attending physician, Case Management, Cardiopulmonary Rehab, Respiratory Therapy, Advanced Heart Failure Team, Heart Failure Coordinator, Chest Pain Coordinator, Care360 Team when applicable with goal to reduce readmission rates as it relates to the CMS targeted diagnosis
The expertise and experiences you’ll need to succeed:
• Minimum qualifications :
• Graduate of an accredited school of nursing
• Associates or Bachelor’s degree in nursing
• Minimum of 3 years of acute care clinical experience
• BLS
• Proficiency in nursing care, knowledge, and skills
• Demonstrates ability to lead, support, and build on current efforts of the readmission reduction program
• Demonstrates initiative, flexibility, integrity, and diplomacy
• Demonstrates strong professional, organizational, and interpersonal skills for effective and creative leadership in working with all levels of the organization including physicians, committees, senior management, and trustees, as well as patients and their families
• Effective communication skills
• Proficient use of Excel, Microsoft Word, Microsoft PowerPoint, and Microsoft Teams
• Experience in working with EMR
• Minimum of three (3) to five (5) years of experience as a Critical Care Registered Nurse.
• Annual continuing education (CME) required including update in Evidence Based Medicine
• Preferred qualifications:
• Acute Care Experience and discharge planning, preferably in a hospital setting
• Working knowledge of current care practices for the diagnostic groups identified by CMS
• Working knowledge of Federal and State and regulatory requirements in hospital accreditation
