Full Job Description Job Title: LVN - Nurse Case Manager
Department: HCS Programs/Nursing
Reports To: HCS Director
FLSA: Exempt
SUMMARY: The Waiver License Vocational Nurse will provide services to individuals with Intellectual Developmental Disabilities enrolled within the Home Community Based Services (HCS) and Texas Home Living (TxHmL) waiver programs, including but not limited to training on medication administration, taking vital signs, seizure precautions and other tasks directed by the RN and individual's physician. The LVN will ensure individual's medication orders are current and accessible to staff/caretakers. The LVN will assist with making referrals and ensuring the individuals meet their plan of care as directed by their physician and the Interdisciplinary team. The LVN will work closely with accounting to ensure most effective billing and on ways to increase efficiency. Services occur within the office, day programs and consumer homes across our four counties. Applicant must have graduated from an accredited LVN program and be licensed by the state of Texas.
ESSENTIAL DUTIES AND RESPONSIBILITIES include, but not limited to, the following:
• Contact the individual at least quarterly to monitor well-being, progress towards outcomes in the IP, and any other concerns important to the individual and LAR
• Communication with local authority at least 60-90 days before individual’s IPC is ready for renewal
• Development and implementation coordination of individual’s plan care
• Coordination and monitoring services delivery, waiver and non-waiver
• Annual documentation completion within 10 days after the IP and IPC meeting, prior to expiration date of plan care
• Conducts special planning conferences within 2 weeks of request by the individual, family, therapist, service coordinator or any other special planning team member
• Completes all special planning conference documentation within 1 of the date of the planning conference
• Completes all quarterly reviews no later than 10 calendar days after the end of the month
• Completes all progress notes by the 10thof the following month
• Ensures all referrals are made within one week of the request, i.e.: therapy and equipment
• Assists the individual in obtaining community services that are not waiver provided. Ensures that HCS Services are provided as a last resort, after private insurance, Medicare, Medicaid and other community services
• Follows up on all referrals and state requests every two weeks and documents the status
• Returns all phone calls ASAP, but within 24 hrs. and will document the conversations in monthly progress notes
• Returns urgent messages within 15 minutes. For critical incidents will submit an Incident Report to the Director of HCS Program Services the following business day and will document the incident in the monthly progress notes. Contact the Director of HCS Programs within 15 minutes to make the Director aware of the emergency situation. If it involves a health or medical issue will also notify RN on call and Director of Nursing
• Communicates with nursing department about consumer in order to keep Director of Nursing abreast of any medical issues with the consumer and request RN assessment when appropriate
• Responds appropriately and in a professional manner to all consumer crises
• Provides training to all providers, as needed, on all training goals and special needs of the individual
• Follows the HCS Consumer Principles for Evidentiary Certification
• Attends a minimum of 12 hours of in-service training relative to this position per year as well as training required to maintain LVN/RN license. A copy of current LVN/RN license is kept on file with HR. Participates in scheduled department meetings and participates as a member of the Advisory Committee
• Demonstrates knowledge and use of all orientation information (Confidentiality, Abuse and Neglect, Universal Precautions, Safety and Accident Prevention, Unusual Incident Reporting and Infection Control)
• All provider progress notes will be read within two weeks of receipt and documented in progress notes
• All consumer training goal data sheets will be read within two weeks of receipt and documented in progress notes
• Nurse Case Manager progress notes will be maintained for each consumer in their caseload documenting current health information and any changes in medication and health status
• NCM will get authorization for all consumer purchases prior to ordering
• Monthly MAR will be maintained and monitored for accuracy, when indicated on RN nursing care plan
• All physician orders will be monitored and updated as needed. Medication education will be given to providers according to RN nursing care plan. RN will be notified of need to assess for delegation of medication administration as needed
• Responsible for initial and annual training of providers, Day Hab/Training Center Staff, school personnel, and/or new group home employees regarding medications/equipment/special needs of individuals including changes in the health status of the individual.
• Responsible for obtaining annual updates of physician orders
• Responsible to follow physician orders when performing specialized procedures
• Responsible to follow universal precautions during treatment of individuals
• When applicable, individuals will be taught self-help skills
• Other duties as assigned by supervisor

EDUCATION and/or EXPERIENCE: Required: Must be currently licensed by the Texas Board of Nursing as an LVN/RN. One year of experience working with individuals with intellectual and developmental disabilities is preferred. Certification in CPR and First Aid is required. Must be familiar with HCS and group home regulations and contract standards for nursing within HCS programs. Must have team building skills. Must exhibit good relational skills when working with individuals and their families. Nursing liability insurance preferred. Should be proficient in word processing and spreadsheets
LANGUAGE SKILLS: Ability to read, analyze, and interpret social work materials, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, individuals, customers, and the general public. REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
MATHEMATICAL SKILLS: Ability to perform basic math skills as they relate to the job
CERTIFICATES, LICENSES, REGISTRATIONS: Current LVN/RN license from the Texas State Board of Nursing, current Texas Driver’s License, proof of auto liability insurance, dependable transportation, and recent TB test required
