Job Duties:
1. To provide clients the necessary services to proceed with alleviating symptoms related to mental health diagnosis.  This may include but not be limited to:
• Weekly individual therapy
• Entries into KOG
• Weekly Therapy Group
• Family therapy
• Initial Assessment
• Comprehensive Assessment
• Initial Tx Plan
• Comprehensive Tx Plan
• Discharge Summary
• Meet with Program Services Director weekly to relay information/client status
• accompany client to local mental health hospital for assessment if/as needed
• CI reports; transfer of information to YSC’s/staff, etc.; doc Watches w/clients – upgrading/downgrading, etc.
• Others as deemed necessary by program services director
• ALPs
• 1294’s
Typical week – 12 – 18 hrs.
Contract rate - $40.00 per hour
2. To document said services related to treatment provision and case management contacts;
3. To communicate with the Program Services Director on a regular basis in order to keep the Agency apprised of the status of each client and family the Contractor is working with;
4. To submit an invoice each pay period for services provided as they pertain to the Residential Treatment Program at the rates agreed upon in the contract
5. To share with the Agency all information obtained regarding a client and family;
6. To maintain strict confidentiality regarding the care and treatment of all clients and families and information in general;
7. Proprietary information as well as confidential documentation are the property of the agency and will be maintained at the agency;
Education/Licensure/Credential requirements:
• Must be at least 21 years of age.
• Must have a valid Kentucky driver’s license.
• Must be independently licensed in the state of KY by LCSW, LMFT, LPCC, PsyD or licensed PhD Psychologist.
• Must have a minimum of two years post graduate experience in supervising treatment of children with emotional/behavioral problems.
• Successful completion of a thorough background check.
