Full Job Description FIELD MEDICAL ASSISTANT
Position Summary:
Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing facilities, assisted living facilities and private residences through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Worker, Clinical and Administrative Support Staff.
We're seeking a talented Field Medical Assistant (FMA) to join our team in the Owensboro, KY area! The FMA enhances the performance of the clinical care team through supporting and facilitating all of the clinical services in assigned facility(s). The Field Medical Assistant will be scheduled to be present at facilities, on days there is an APP in the facility. On days there is no APP in a facility the FMA will be assigned to provider support remotely.
Essential Duties and Responsibilities:
Provider Support:
• Ensure that the clinical provider in the assigned building is able to focus on taking care of their patients for the day
• Ensure that each patient is seen as medically necessary per clinical protocol.
• Assist with collection of facility acute books each morning. Sort all clinical paperwork needed for SNF providers (NP/PA or Physician) in the building each day. Ensure paperwork completion each day
• Reviewing the facility acute books each morning, assist with any day-of changes to the scheduled visits due to acute needs, hospitalizations, admissions, or discharges
• Prepare clinical documentation and note prep for the NP/PA in the building each day
• Post clinicians daily patient schedule by 5pm, the day prior to patients being seen. Add daily schedule in EHR and Microsoft Teams
• Complete opening of encounter note
• Perform medication reconciliation on all scheduled patient encounters
• Upload lab results and facility chart data on all scheduled patients each day into provider notes
• Scheduling rosters should be updated daily to ensure accuracy.
• Submitting patient status changes daily
• Document appropriate time for CCM services
• Host telehealth sessions for any needed providers, both routine and acute visits
• Be available to all providers in the building that day to assist with paperwork, facility communication and provider needs
• Track open notes for provider and assist with necessary information needed to facilitate closing of notes
Facility Support :
• Attendance is expected in assigned building based on days and time provider is scheduled
• Checking and triaging facility acute books each morning and adjust provider schedule as needed.
• Share patient schedules with key facility staff as needed
• Be first line for any triage needs that can be communicated out the appropriate provider
• Assist with triage documentation as needed
• Ensure that all needed paperwork for the facility is routed to Eventus providers or appropriate office staff
• Assist with consents for any services needed
• Attend morning stand up meetings to report any needed information from the facility to Eventus providers and vice versa.
• Assist with discharge planning of clinical services as needed
• Ensuring check in and out each day with facility staff regarding patients seen, status, etc.
Patient Support :
• Maintain patient confidentiality at all times
• Check all forms of communication several times daily to ensure patient needs are met
• Inform patient and family of recent visits, new orders and defer to provider when needed
• Communicate medication changes made that day, when requested by the provider.
• General updates to family as requested. These conversations will be generic and based off the provider’s documentation.
• Reviewing medication list with family to clarify/confirm accuracy.
• Obtaining additional medical history as appropriate, specialty services/names of providers, etc.
• Communicate transitions of care to CCM team to enhance patient care and education
Additional Duties and Responsibilities:
• Field Medical Assistants are expected to be knowledgeable and adhere to Eventus WholeHealth standards, policies and procedures and outlined in employee handbook including HIPAA rules and regulations
• Medical Assistants should adhere to the Eventus WholeHealth dress code policy i.e. clean, wrinkle-free, matching scrubs with appropriate, closed-toed footwear and name badge.
• When facility duties are completed ahead of scheduled work day, report to SNF Support Manager for any additional assignments
Qualifications/Education:
• High School Diploma or equivalency
• Certified Medical Assistant degree a plus
• Typing speed of 45wpm or greater
• Medical Scribing experience preferred
