This positions will direct and coordinate the overall functions of Post-Acute Care (PAC) Department. Work together with senior leadership to build and maintain efficiencies within the company to assure growth and productivity. Ensure the PAC product is positioned for growth, increased market share, and operates in an efficient manner, maintaining clinical superiority.

• Serve as a core leader among SN leadership and product management in meeting company objectives.
• Prepare and implement business plans, operating budgets, and product roadmaps.
• Report business performance, KPI, financial reports up to senior leadership.
• Evaluate and determine the various departmental needs, issues, and problem areas, and provide recommendations on appropriate course of action to executive staff.
• Manage PAC staff; implement and maintain policies, procedures, workflows, processes and system improvements to increase the effectiveness and efficiency of the PAC Division.
• Coordinate with Provider Relations Department to recruit PAC facilities in order to maintain growth in network participation.
• Identify and actively pursue new product offerings within core markets and emerging, new markets
• Maintain a high level of knowledge proficiency regarding industry trends, regulatory matters, reimbursement guidelines, and other environmental factors.
• Manage to maximum monthly financial throughput and report results up through finance and leadership.
• Assist and educate the Case Coordination department in all matters related to scheduling to PAC network facilities, or PAC facilities outside of the network.
• Assist and educate the Case Coordination department with one-time agreements for out of network facilities.
• Promote and maintain good inter-departmental communication regarding daily activities and procedure.
• Oversee goal setting, performance evaluations, salary reviews, training, hiring and other programs for PAC staff members.
• Hire, retain and develop department employees, supervisors and managers to ensure productivity and meet high levels of customer service standards.
• Promote company growth, efficiency in productivity and quality and encourage employees input on how to achieve these standards.
• Effectively identify and develop intra-company means of cross pollinating the product.
• Interface with customers to promote company awareness; research client/provider problem/issues and resolve or relay information to executive staff for review and action.
• Understand the definition of quality and position the business to be able to report on measurable outcomes.
• Promote the program broadly throughout the industry through prepared educational content and brand approved social media presence.
SUPERVISORY RESPONSIBILITIES:
• Mentor, train, and supervise team members directly and indirectly, review their work and provide effective constructive feedback.
• Ensure all team members understand, are trained in, and comply with Paradigm’s security requirements and policies.
• Ensure all team members have the minimum level of IT system access required to effectively complete their Paradigm responsibilities

Requirements
• College education with preferred 7-10 years executive Post-Acute Care experience.
• Comprehensive understanding of all company operations and the future direction/growth objectives of the firm.
• Outstanding deductive reasoning, analytical, judgment and decision-making skills essential.
• Strong ability to manage and coordinate the workflow of a diversified work force.
• Keen awareness of the importance of keeping executive staff informed of all departmental needs/issues.
• Excellent written/verbal communication and listening skills necessary.
• Proficient computer skills as well as proficient in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint and Access).
• Bilingual (English/Spanish) a plus.
