**Description** The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. **Responsibilities** The Field Care Manager, Behavioral Health 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. + Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. + May create member care plans. + Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. + Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. + Follows established guidelines/procedures. + This role will require local travel to see members within a 30 miles radius from where you reside. Travel could be to a members home, PCP Office, Local Hospital or other clinical settings + Will be required to report to the local Market office for meetings (see additional Information) **Required Qualifications** + Master's Degree in Behavioral Health, Social Services or Human Services Applicable State licensure in field of study: **LCSW, LSW, LMHC, LMFT** + 3-5 years' Experience working with members with Behavioral Health and Substance use needs and crisis. + Experience assisting members with social determinant of health needs: Housing, transportation, childcare, food insecurities, financial needs etc. + Ability to navigate Microsoft Excel, Outlook, Power Point and Word + Additional information: This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. + Associates working in the State of Florida will need ACHA Level II Background clearance. + This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits **Work At Home Requirements** + Must have a separate room with a locked door that can be used as a home office to ensure you and your members have absolute and continuous privacy while you work. + Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required. **Travel** + This role will require local travel to see members within a 30 miles radius from where you reside. Travel could be to a members home, PCP Office, Local Hospital or other clinical settings + Some locations that will be covered are Palm Beach/ North Broward County on the East Coast of Florida **Preferred Qualifications** + 1 year Case Management + Medicaid Program experience + Experience with case management, discharge planning, crisis intervention and patient education for adult acute care + Experience working with Child, Adolescent and Adult + Experience working with Chronic homelessness, Social determinants of health + Experience working with members/patients with medical and mental health co-morbidities + Managed care experience + Certified Case Manager (CCM) **Additional Information** **Onsite Travel** In order to support the CDC recommendations on social distancing and reduce health risks for associates, members and public health, Humana is deploying virtual and video technologies for all hiring activities.
