Role:
The Care Access Nurse supports the needs of patients/families being admitted to services by ensuring clinical information is adequate for a safe and effective admission and to facilitate obtaining clinical record details to support eligibility for home health or hospice services. Acts as a resource for referral sources and patients/families being admitted to Chapters services. Operates with a sense of urgency to ensure speed and accuracy to facilitate timely admission to services while adhering to Organizational expectations of productivity and efficiency.
Qualifications: • Current Florida license as RN, current active single-state out-of-state nurse license, or current active multistate/compact nurse license. • Minimum of two (2) years' RN experience including one (1) year of hospice nursing experience; admission experience preferred • Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations • Able to effectively solve unique problems as they arise or identify when to consult the supervisor; exceptional problem-solving abilities • Knowledge of current Hospice and Homecare admission criteria, Medicare, Medicaid, Florida state and Federal regulations • Knowledge of Chapters service lines/programs, including, but not limited to, Hospice, Palliative Care, Home Health, etc. • Skill and experience in the Medicare Hospice benefit and regulations • Demonstrated knowledge of the Hospice Local Coverage Determinations (LCDs) • Excellent telephone/verbal, interpersonal, customer service, and written communication skills • Strong commitment to teamwork and customer service, both internally and externally • Strong/proven data entry and general computer skills • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system • Certification from HPNA obtained within two (2) years of taking the position • Excellent critical decision making and problem solving skills • Ability to use equipment with visual and auditory mechanisms • Ability to effectively communicate (written and verbal) in English with customer service focus • Flexible schedule to meet the needs of patients • Ability to manage the emotional stress of working with and providing information and support to terminally ill patients and their families Competencies: • Satisfactorily complete competency requirements for this position. Job Responsibilities: • Acts as the in-office clinical liaison that represents the Organization via telephone for referrals for patient care. Interacts frequently with referral sources, other members of the referral center staff, and all others necessary to facilitate the start of patient care. Referral source interactions include, but are not limited to, working closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families, and the general public. • Responsible for receiving/processing referrals from the Patient Access Services Team, handling complex referrals, and ensuring that admission information is ready for the Admission Nurse in a timely and consistent manner. • Enters referral information in the system and creates initial orders to facilitate the start of care ensuring needed complex patient orders are in place (IV, wound care, medications, etc.). • Ensures adequate patient information is available for certifying Physician or APRN for hospice patients without delaying admission. Provides updates to the certifying Physician, as directed by department protocol or as requested. • Coordinates with the Professional Relations Team regarding referral information. • Provides excellent customer service with referral sources and patients/families and ensures all processes are completed accurately and timely. • Identifies objections, concerns or other entry barriers and provides information in response to address those issues. • Describes and explains services to potential patients/families while putting them at ease. Proactively offers alternative services to meet patients' needs and actively refers them to the Clinical Navigation Team. • Makes home or facility visits for admission visits, as directed by management, in periods of low staffing or case surges. • Performs other duties as assigned.
