Services provided by the SPRN will include, but are not limited to, general patient care, assistance with procedures, documentation of activities and patient/family education. The Team Lead also performs job duties, which are specific to department in which they are working; these duties will require special training. The will provide care utilizing established principles and ethical standards as well as policies and procedures established by Deaconess. The Team Lead is responsible for the leadership, coordination and management of the daily operations, front desk personnel and clinical personnel of the department they are over. The Team Leader assures that the practice functions efficiently to support the physicians and/or midlevel providers in providing safe and compassionate patient care. This position is for Lead Clinic Practice RN III (Team Lead) for the Owensboro office. #Dr. Devabhaktuni, Dr. I. Siddiqui, Dr. Balagna, and one APP will be seeing patients in the office three days per week. #This person will also be the Team Lead at the DSP#Henderson office.# Job Duties#include the following, other duties may be assigned: The Team Lead must be able to demonstrate knowledge of growth and development to provide patient care appropriate to the age of the patients served The Team Lead must be able to successfully demonstrate the staff requirements as specified in the department#s competencies and Scope of Service (if applicable), and meet/exceed standards as identified below and meet the requirements of the applicable state Nurse Practice Act. Reviews patient data prior to each office visit for relative clinical updates, laboratory, x-ray and other test results. When rooming patients, obtains medical and social history, chief complaint and/or history of present illness, completes appropriate assessments (e.g., vital signs, height, weight), and reviews and updates the medications list. Documents all data obtained in the Electronic Health Record (EHR). Prepare patient for examination, set up any necessary equipment. Assist physician with procedures and examinations, which are within scope of practice of the RN as ordered by physician. Demonstrates sterile technique during procedures and adheres to safe operating guidelines for all departmental equipment. Provides accurate and timely procedure scheduling with minimal errors. Administers nitroglycerin, oxygen, and other in-office medications as ordered by the physician. Confers with the patient and physician regarding scheduling priorities for acute and routine visits. Utilizes the EHR to enter and schedule all patient referrals, diagnostic services, tests, procedures, etc., as ordered by the physician. Obtains and documents prior authorization as needed. Educates patient regarding preparation instructions as appropriate for testing and invasive procedures. Receives and responds to telephone requests from patients under the direction of the physician and/or midlevel practitioner. Documents the request, contact information, physician or midlevel response, patient follow up and any other pertinent information in the EHR. Medications, pharmacy and allergies should be reviewed and updated during each patient telephone encounter. Make sure we are doing this # it#s in our policy and we will get hit for any ISDH complaint. Implements and positively reinforces physician/provider orders and/or treatment methodologies. Provides practice-specific care to meet the needs and behaviors of patients based on the patient#s age and assessed level of growth and development for all stages of life. Cleans the patient exam rooms and maintains patient flow through the office by efficiently utilizing exam rooms. Maintains an adequate amount of supplies in each exam room. Restocks department supplies, monitors supply usage, and requests to have supplies reordered in a timely manner. Ensures that all medications, are current and in a secure location Maintains daily control logs for Compliance including but not limited to daily refrigerator/freezer temperatures, waive testing controls, and daily disinfecting of exam tables, work surfaces and equipment. Adheres to all policies for compliance with ISDH, OSHA, CLIA, HIPAA, HFAP and any other appropriate regulatory agencies. Utilizes universal precautions in handling infectious/hazardous waste material. Maintains current knowledge base and stays current with healthcare trends by reading professional journals/literature, attending CME#s, and completing all required Web Inservices, and seminar/conference attendance as approved by the Manager. Reads and replies to educations/informational e-mails in a timely manner. Attends staff and other meetings as required. Performs other duties as assigned. Complies with Standards of Employee Conduct as specified by Policy 45-19. Maintains astute physical assessment skills of Cardiovascular and Pulmonary Systems Utilizes nursing judgement to determine severity of patient problems, responding to urgent situations and determining what interventions are needed to assist the patient consulting with MD or mid-level as indicated. Responsible for communications with referring physicians and other providers. Communicates with hospital based staff to assure effective, efficient care to the patient, providing excellent transitions of care to the ambulatory setting. Reviews incoming correspondence, results, and reports to prepare for physician review and anticipates physician or midlevel provider actions. Receives patient calls and elicits questions, concerns, and data to present to physician or midlevel provider for response. Relays physician or midlevel response to patient, answering questions and offering clarification. Develops triage procedures and prompt response to incoming phone calls; documents all conversation by way of encounters or telephone notes. Reviews and evaluates labs and test results, determining the significance of the results and what appropriate follow up steps are needed based on clinical protocols established in collaboration with the cardiologist or mid-level, including sharing results with the patient, when appropriate, and advising the patient of the plan of care. Arranges for hospital admissions, procedures, labs, and follow up appointments with assistance of ancillary personnel and instructs patients accordingly. Calls in prescriptions/refills to pharmacies under physician direction. Educates and counsels patient, patient#s family and/or significant others regarding cardiac disease, risk factors, diagnosis, treatments, procedures, lifestyle modifications and medications (including how to take dose amounts, and potential side effects). Prepares, creates, and gathers materials to facilitate education, as needed. May travel to other offices or clinics as scheduled. Additional Administrative Duties: Performs initial staff interviews, including administration when necessary Follows Deaconess Health System and DSP P#Ps and counsels staff, consulting Human Resources as necessary Maintains staff schedule ensuring adequate coverage for clinic operations Maintains employee payroll in DHS timekeeping system Performs employee competencies at the introductory evaluation period and annually thereafter Completes introductory and annual evaluations in DHS performance manager system according to health system policy Conducts staff meetings with departmental area Education and Experience Graduate from an accredited RN school of nursing. Experience preferred but not required. Certificates, Licenses, Registrations Current state appropriate licensure as a Registered Nurse. BLS CPR provider certification required. Why Choose Deaconess?
