ORGANIZATIONAL SUMMARY:
The CMS Referral Center is fully funded by the Florida Department of Health and contracted with USF Health.  The Referral Center contract provides care coordination and professional health care support services for CMS.  Center staff liaison with health care facilities in the Tampa area for CMS care coordination and administrative services, CMS Hemoglobinopathy services, Brain and Spinal Cord Injury Program care coordination services, Craniofacial and Cleft Palate care coordination and administrative services, and Medical Foster Care Program care coordination and administrative services.  There are approximately 20 staff in this unit (mostly comprised of RN's).
POSITION SUMMARY:
The Case Coordinator is responsible for providing social work interventions to medical foster care children and their families.  In this role, the Case Coordinator provides assessment of social needs, social services case planning, and counseling for patients and their families.  Travel from site to site and/or for patient home visits is required for this position.  This position is grant funded.
RESPONSIBILITIES:
• Performs initial and ongoing psychosocial assessment and counseling to medical foster children, their birth families, and foster families.
• Coordinates all day-to-day referrals, intake activities, placements and discharges for the Medical Foster Care Program.  This includes arranging staffing and medical parent-child specific training.
• Conducts medical parent support activities including assisting with Medicaid provider rules and regulations, organizing and providing medical parent support activities, and assisting in obtaining community resources.
• Assures program standards, rules and policies are adhered to and conducted ongoing review of all medical foster care records.  Participates in quarterly and annual program reviews.
• Participates in training and educational activities for Medical Foster Care Program staff and parents, and other program staff and health care professionals.
• Coordinates placements, changes, and discharges with Department of Children and Families staff.
• Performs activities related to medical parent recruitment, conducts pre-service and in-service training, coordinates initial medical parent approval process, ongoing medical parent evaluations, and renewals of medical foster care homes.
• Acts as liaison with Department of Children and Families programs and other community agencies.
• Maintains all administrative records including one for each child and parent.
• Compiles information and prepares reports as required and maintains all data and records for information collection.
• Other duties as assigned.
POSITION QUALIFICATIONS:
MINIMUM:
• Each position assigned to this classification must require specific training at the Bachelor's degree or Master's degree level in Education, Special Education, Psychology, Social Work, or other field appropriate to the program content.
• Bachelor's degree candidates must have at least two years of applicable experience.
• The Master's degree may substitute for the required experience.
• Specific certifications or licensures may be required or preferred for certain program areas.
PREFERRED:
• Master's degree preferred in public health, social work, special education or any related clinical discipline.
